Post-stroke depression(PSD) is a depressive symptom whichoccurs in a variable extent in stroke patients and lasts more than twoweeks or longer. PSD is a common complication of cerebral vasculardisease, which can impede the process of rehabilitation, increase themedical cost and influence the life quality. Therefore PSD receives alot of appreciations. But because of the absence of the peculiarbiological determinants and diagnostic standard, in clinical practiceonly a minority of the patients are diagnosed and even fewer aretreated. The purpose of this study was to investigate the controversialrisk factors and determined the content of total cholesterol,triglycerides, high-density lipoprotein cholesterol and low densitylipoprotein cholesterol(TC, TG, HDL-C and LDL-C). So we canprovide the biological determinants and the predictive factors todiagnose the disease early.50 patients after stroke were be studied in the department ofneurology, The 1st Hospital of Jilin University from October 2003 toOctober 2004, whose course were shorter than one-month, havingpossession of receptive speech comprehension and the ability tomaintain attention adequate for participation in the study interview.According to the Hamilton Depression Rating Scale, we divided thepatients into two groups, the PSD group higher than 17 and non-PSDgroup. The healthy volunteers were selected as the control group,whose sex, age, profession could compare with the stroke groups. Thesingle factor analysis and multiple factor analysis with the logisticmethod were carried out on the thirty-six relative factors. Selecting thepatients and severe corpus complications, Contents of TC, TG, HDL-Cand LDL-C of the patients'serum were determined by fluorometry toassess the relation of the changes of central transmitters and theincidence of PSD, also as to the relation of lesion location and theseverity PSD.The result of this study showed that the incidence of depressionwas 46.0%, among which the minor and moderate and majorincidences were 22.0%, 18.0% and 6.0%. We analyzed the thirty-sixfactors by the single factor analysis and found that among which, thetwelve factors including family harmony, the person who attended tothe patient, character, lesion location, lesion volume, the AP amount,the ability of life, intellect scoring and social support have significantdifference between the PSD group and the non-PSD group. Then weanalyzed the thirteen factors with the logistic regression method andconcluded that they had the close contact with the PSD. A discontentwith the person who attended to them, the left anterior lesion and poorsocial support, have the high PSD incidence and the high mark of thePSD severity. On the contrary the PSD incidence and mark of the PSDseverity were low. From the relation of lesion location and PSD wefound that the PSD incidence of patients with left lesion was 56.5%,the PSD incidence of patients with right lesion was 26.1%, while theleft anterior lesion and the right anterior lesion were 47.8% and 21.7%respectively. The difference of PSD incidence between anterior andposterior, left and right, left anterior and other locations weresignificant, which showed that PSD was associated with lesionlocation, especially left anterior lesion.Many scholars think the stroke lesion destroyed the NE, 5-HTneurons and their gateways and decreases them, which caused thedepression. Among 50 stroke patients, the patients with PSD were 23,those without PSD were 27, and the control group were 25. The serumcontent of lipid were: PSD group: TC 4.1±0.9mmol/L, TG2.02±0.26mmol/L, HDL-C 1.02±0.23mmol/L, LDL-C 2.6±0.8mmol/L;non-PSD group: TC 4.5±0.9mmol/L, TG 1.7±0.4mmol/L, HDL-C1.06±0.23mmol/L, LDL-C 2.7±0.9mmol/L; control group: TC4.9±0.7mmol/L, TG 1.55±1.05mmol/L, HDL-C 1.32±0.35mmol/L,LDL-C 2.8±0.9mmol/L. We could see that serum TC concentration inpatients group were significantly lower than those in controlled group.Significantly positive correlations existed in patients group betweenserum TC and HDL-C levels respectively.From the present result, we inferred the relationship betweenplasma monoamine neurotransmitters and serum lipid concentrationsin depressive patients. the content of serum TC and HDL-C levels in...
