Objective: To conclude the experience of diagnosis and treatment of calcaneus fracture .discuss the necessarility of the choice of the style of open reduction and internal fixtion, bone grafting, primary arthrodeses and disposing of open fracture . Methods: From March 2000 to December 2004, 101 calcaneal fractures in 89 cases, including 8 of type I, 34 of type II, 36 of type III and 16 of type IV according to Sanders classification, were treated by close and open reduction to restore the posterior articular facet, subtalar facet, Bohler angle and Gissane's angle, following internal fixation by plastic titanic alloy plate or Kirschner's wires with or without bone grafting. Fractures were followed up for 1 to 28 months, 11.5 months on average; joint function were evaluated according to Maryland Foot Score System . Results: There was no difference with type II and type III fractures fixed by plate or Kirschner's wires in open reduction (P>0.05);there was a clear statistically significant superiority with type II and type III fractures treated with bone grafting when compared without bone grafting (Sanders II P<0. 05, Sanders III P<0. 01). Conclusion: On the basis of our result, we recommend that type II and type III fractures be treated with open reduction , bone grafting and fixed by Kirschner's wires .
