Objective: To investigate the expression and clinical significance of E-CD in human transitional cell cancer of bladder; to probe the cause of the abnormal expression of E-CD on the level of mRNA; to investigate the expression and clinical significance of urinary soluble E-CD in human transitional cell cancer of bladder.Materials and Methods:1, To investigate the expression of E-CD in human transitional cell cancer of bladder:56cases of specimen of transitional cell cancer of bladder and 10 cases normal tissue of human bladder mucosa came from the pathological department of Qilu hospital of Shandong university. We analyzed all these specimen with anti-E-CD monoclonal antibody by the method of immunohistochemistry, and the result was analyzed according to the intense of staining. We analyzed the relationship between the expression of E-CD and the biological propert of the tumor with the statistical method of X~2.2, To investigate the expression of E-CD mRNA in human transitional cell cancer of bladder: 24 cases of specimen of transitional cell cancer of bladder and 16 cases normal tissue of human bladder mucosa came from the urological department of Qilu hospital of Shandong university. The level of E-CD mRNA was measured by the method of RT-PCR. We analyzed the relationship between the expression ofE-CD mRNA and the biological propert of the tumor with the statistical method of rank test.3 ^ To measure the expression of soluble E-CD in human transitional cell cancer of bladder: the soluble E-CD was measured by ELISA in the urine of all 59 cases from the urological department of Qilu hospital of Shandong university. We analyzed the relationship between the expression of soluble E-CD and the biological propert of the tumor with the statistical method of rank test.Result:1 x The expression of E-CD: there is all preserved expression in 10 cases normal tissue of bladder; in human transitional cell cancer of bladder, the rate of preserved expression as followes: Gl: 92.86°/ T2-T4: 46.34%, there is significant statistical difference in them, PO.01; in cases of first occurring(3] cases) and recurrence(25 cases), the preserved expression rate is 70.97% and 40.00%, there is significant statistical difference in them, PO.05; the recurrence rate in cases of preserved expression(32 cases) and loss expression(24 cases) is 21.88% and 58.33%, there is significant statistical difference in them, PO.01; the preserved expression rate in mono-occuring cases(26 cases) and multi-occuring cases(30 cases) is 57.70% and 56.67%, there is no significant statistical difference in them, P>0.1; the preserved expression rate in cases <2cm(18 cases) and cases >2cm(38 cases) is 55.56% and 57.89%, there is no significant statistical difference in them, P>0.1.2> The expression of E-CD mRNA: the expression of E-CD mRNA in cases of human transitional cell cancer of bladder and normal bladder mucosa is 0.5693 ± 0.3911 and 1.1205 + 0.3850, there is significant statistical difference in them, PO.05; in Gl, G2 and G3 cases the expression of E-CD mRNA is 0.8503±0.3964, 0.3481 ±0.1817 and 0.2848±0.1369, there is significant statistical difference in Gl vs G2 and Gl vs G3, PO.05, there is no significant statistical difference in G2 vs G3, P>0.05; the expression of E-CD mRNA in superficial cases and invasive cases is 0.9623 ±0.3686 and 0.3705 ±0.2168; there is significant statistical difference in them,P<0.01; in cases of first occurring and recurrence, the expression is 0.7441 +0.3741 and 0.2739 + 0.2005, there is significant statistical difference in them, P<0.01; the expression in mono-occuring cases and multi-occuring cases is 0.5577 + 0.4676 and 0.5819±0.2746, there is no significant statistical difference in them, P>0.1; the expression in cases >2cm and cases <2cm is 0.6106±0.4494 and 0.4078±0.2790, there is no significant statistical difference in them, P>0.1.3^ The expression of soluble E-CD: the expression of soluble E-CD in cases of human transitional cell cancer of bladder, urinary benigh disease and normal control is 7.114 ±7.515 mg/molCr, 1.602 ±1.109 mg/molCr and 1.306 ± 1.249mg/molCr, there is significant statistical difference in bladder cancer cases vs urinary benigh disease cases and bladder cancer cases vs normal control cases, P<0.01, there is no significant statistical difference in urinary benigh disease cases vs normal control cases, P>0.05; in Gl, G2 and G3 cases the expression is 2.338± 1.277 mg/molCr, 5.991 + 2.441 mg/molCr and 11.813 ±9.033 mg/molCr, there is significant statistical difference in Gl vs G2 and Gl vs G3, PO.01, there is no significant statistical difference in G2 vs G3, P>0.05. the expression in superficial cases and invasive cases is 4.530 ± 2.524 mg/molCr and 7.443 ± 7.088 mg/molCr; there is no significant statistical difference in them, P>0.05; in cases of first occurring and recurrence, the expression is 3.724+1.892 mg/molCr and 10.497 ± 7.471 mg/molCr, there is significant statistical difference in them, P<0.01; the expression in mono-occuring cases and multi-occuring cases is 5.423 ± 4.387 mg/molCr and 7.103 + 7.091 mg/molCr, there is no significant statistical difference in them, P>0.05; the expression in cases >2cm and cases <2cm is 4.579± 2.815mg/molCr and 8.007 ± 7.518mg/molCr, there is no significant statistical difference in them, P>0.05.Conclusions:1?. The expression of E-CD is very closely linked with the biological behavior of the transitional cell cancer of bladder, and the low expression indicates the high malignance, the result has an important significance in clinical practice.2> The low expression of E-CD mRNA indicates the high malignance of tumor, this result coincide the expression of E-CD. It indicates that the lower expression ofE-CD is related to the lower expression of E-CD mRNA.3 > The expression of urinary sE-CD is very closely linked with the biological behavior of the transitional cell cancer of bladder, and the high expression indicates the high malignance. The measurement of sE-CD in urine has an important significance in the early diagnosis of transitional cell cancer of bladder and the estimation of its biological behavior.
