Objective: By observing the drug use of the treatment under three-year-old children catching common cold and diarrhea in study areas after intervention, to find out whether intervention effect evaluated from provider is all the same with it from demand side; The main influential factors on irrational use of drugs was explored and put forward some reasonable policy suggestions. Methods: The sample is selected from those have been continuously in the project area from 1999 to 2001, it cover 14 counties among four provinces. The basic family situation of the under three-year-old children, the treatment of their common cold and diarrhea and the basic situation of the provider are analyzed by descriptive statistics. Using logistic regression analysis gets the main influential factors on the injection treatment for children common cold and diarrhea. Anova analysis and Chi-square analysis are used to analyze which factors are the main factors that influence the irrational use of drugs at demand side. Results: It shows that almost half of the children catching common cold and diarrhea are treated in the village clinic and the town hospital is on the second place. Almost half of the children with common cold and diarrhea are treated by injection and 20% of which are intravenous injection. Also 20% of all have the wrong water and food forbidden treatment. The changing tendency of the treatment of the common cold and diarrhea are similar. Comparing the situation between 2000 and 2001, the percentage of the muscle injection has decreased, but the percentage of the intravenous injection has increased and the use of ORS has decreased. The family character of the use of injection both common cold and diarrhea are similar, including the number of the family member, whether there is TV set, the sex of the child and the seniority among siblings. By investigating the provider side, it shows that the use of the injection has decreased remarkably after intervention. In town hospital, the use of the muscle injection has decreased while the intravenous injection has increased in 2000 and 2001. Both the use of the sterilize equipment and the ability of the village doctors has improved. The situation of drug purchasing in village clinic becomes more nonstandard. Conclusion: The situation of irrational drug use in the treatment of under three-year-old children with common cold and diarrhea in study area of western rural China is still serious after intervention. The intervention on irrational drug use achieves certain effect evaluated from providing side, but the effect is not obvious if evaluated from demand side. It should be attached importance to intervention on demand side, and health education is the mainly available way.
