Experimental Study On Skin Growth Promoting Ointment In A Rabbit Model Of Chronic Cutaneous Ulcer Healing

The chronic cutaneous ulcer is a common disease, but people has paidmuch attention to it because of its seriousity and increasing incidence.Topical treatment is one of the main methods,and Chinese medicine is richin clinical experience on the wound healing. Skin Growth PromotingOintment was a pure Chinese medicine prescription, which was composedby beeswax as a basic remedy. According to the vast amount of clinicalresearches, it was demonstrated that Skin Growth Promoting Ointment hasthe remarkable therapeutic effects to various chronic cutaneous ulcers. This present study is to investigate whether Skin Growth PromotingOintment(beeswax)can promote wound healing and its mechanism and tofind a new remedy for treating the chronic cutaneous ulcer and provide thepharmacological basis of the new remedy. Methods: Three full-thickness skin wounds of 2.54cm2 were incised in the backof each diabetic rabbit induced by the alloxan,and the wound were treatedwith Skin Growth Promoting Ointment(SGP),MEBO or Matrix(LiquidParaffin ) respectively.The wound healing effect was assessed bymorphology and the measurements of wound area and the healing time ofeach group;The specimens were obtained at the 4th,9th and 14th day afteroperation and the histology was observed;The expression of vascularendothelium growth factor(VEGF),proliferative cellular nucleolus antigen(PCNA) and epidermal growth factor(EGF) were investigatedquantitatively by means of immunohistochemistry and image analysis. Results: The wound area reduced obviously in SGP group after 8 days and Thehealing time of granulation wounds was shorten obviously for about18.9 ± 1.4 days than that of Matrix group (P<0.05). Histopathologyshowed the number of fibroblasts which were 45.7± 9.9 and 91.2± 7.4, andnew blood capillaries which were 7.15± 2.20 and 15.12± 3.10 in SGPgroup were obviously improved as compared with Matrix group(P<0.05 orP<0.01) at the 4th and 9th day after operation. With immunohistochemistryand image analysis, the expression of VEGF(173.8± 6.3,146.9± 8.3 and158.4 ± 8.0),PCNA(172.8 ± 7.6 , 163.1 ± 8.8 and 168.5 ± 5.6) andEGF(171.7± 5.1,159.8± 7.3 and 165.1± 7.6,) in SGP group were higherthan Matrix group at the 4th,9th and 14th day after operation(P<0.05). Inaddition,the results showed that SGP and MEBO which was used aspositive control drug had the same pharmacological potency with nostatistically significant difference in the above experiments(P>0.05). Conclution: Skin Growth Promoting Ointment could cure chronic cutaneous ulcersand the mechanisms of SGP on wound healing were probably related to:SGP could promote the fibroblast hyperplasia and new blood capillaryformation,and it also could upregulate the expression of VEGF,EGF andPCNA in the wound significantly. The effect of SGP on wound healingwas in line with MEBO.
