[Objective]In theoretical aspect, to establish recognition of the etiologicai factor, pathogenesis and treatment by differentiation of syndrome of Severe Acute Respiratory Syndrome(SARS) according to theory of Seasonal Febrile Diseases.In clinical aspect, to analyze the symptomatic characteristic of patients who were differentiated as weifen and/or qifen syndrome; to approach the curative effect of treating SARS patients of weifen and/or qifen syndrome with Traditional Chinese Medicine (TCM)-dominant therapy. [Methods]Theoretical exploring : To summarize Western medical literatures of SARS, to summarize Traditional Chinese Medical recognition of the name, etiologicai factor, pathogenesis and differentiation of syndrome of SARS; to review researches about the treatment of SARS,Clinical exploring: clinical manifestation (including symptoms and some indices of laboratory examination) of sixty patients were analyzed statistically, all the sixty patients came from The First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine and were defined as weifen and/or qifen syndrome according to symptom differentiation system of wei-qi-ying-xue. Characteristics of symptoms have been summarized.Sixty patients (Group A) were treated with Traditional Chinese Medicine-dominant therapy. The basic decoction and modify had been formulated according to the principle of "dispelling wind, clearing away heat and toxins, removing dampness" . Indices of the near curative effects included: the average defervescence time (ADT) , the average hospitalization time(AHT) , the average time of inflammation-shadow on chest radiography initially being absorbed, time for obvious absorption or complete absorption of shadows. Theseindices were analyze statistically. For many reasons, it is difficult to design a random control group to complete the randomized controlled trial. Therefore, we searched a historical control group (Group B) of better comparability to compare curative effect with Group A.A follow-up study has been undertaken in partial patients of Group A six months after they were discharged. Major items of follow-up study included: blood analysisx urine analysisN some indices of serum biochemistry and immunology> X-ray of femoral articulation> magnetic resonance of femoral articulation spiral computed tomography of thoracic region etc. According to results of the follow-up study, prostecdtive efficacy has been estimated. Some results have been compared with those of contemporary-health volunteers' , another results have been compared with the corresponding former results (results of duration of hospital). [Results]Characteristics of syndromes: sixty patients of weifen and/or qifen syndrome possess evident characteristics of wind-heat % dampness and pyretic toxicity.Immediate efficacy: All of the sixty patients had been cured and discharged. The ADT was (4.1 + 1.9) d, the average time of inflammation-shadow on chest radiography initially being absorbed was (4.2±3. 2)d, average time for obvious absorption or complete absorption of shadows was (8. 4±4. 6)d, the AHT was (9. 0 ±4.9) d. The cure rate was 100% and transmission into yingfenor xuefen didn' t occur, Acute Respiratory Distress syndrome (ARDS) didn' t occur. Without one case was treated with immunoglobulin or antiviral drugs, only one case was given continuous positive-pressure oxygen-supply mask, only five cases were given Dexamethasone for a short time. Pulmonary fibrosis and bone necrosis didn' t occur in the duration of hospital stay. Advantage of Group A is obvious as comparing with Group B.Prostecdtive efficacy: Serum IgG> IgM, C3% C4 and T cell subgroup(CD3+, CD4+, CD8+ cells and NK cells) in peripheral blood have been checked in thirty-one cases. Results of these indices have no evident difference as comparing with those of contemporary health volunteers' . Only few cases have lightly abnormality in serum ALT. AST-. BUN> CREA in the thirty-three patients who have undertaken these checks. Mean value of ALT> AST and CREA of these thirty-three patients is much lower than their former ones (results during... |
