IntroductionSevere acute pancreatitis is a very dangerous emergency case of the alimentary system. It' s severest complication is systemic inflammatory response ( SIRS) who lead to multiple organ failure ( MSOF). TNF - α, as a multieffect cytokine, educe multiple action in the proceed and development of acute pancreatitis. TNF — α has a cell toxic action to endotheliocyte, increase adhesion of heterophil granulocyte to the endotheliocyte, increase vasopermeability, finally lead to MSOF. One of the pathogenesis of acute pancreatitis is angiospasm and intravascular coagulation lead by microcirculation disturbance. If the Inflammatory reaction is fierce, it happens to the blood vessel of other organ too. Because of the special contact with pancreas in anatomical position, physiologic function and haemodynamics, the liver is easy to be involved during the process of acute pancreatitis. Our research investigate the protection of Urokinase to the liver in acute pancreatitis through making models of SAP in rats, treating them with Urokinase and observing the volume of blood flow of the liver and the change of TNF - a level .Materials and Methods192 male Wistar rats were randomized into control group ( C) , pancreatitis group(P) and treatment group (T). Each group was divided into subgroup A and B. The volume of blood flow of liver was measured in subgroup A while blood samples and liver tissues were collected to observe the changes of TNF - αlevels and pathologic changes of liver in subgroup B. Each subgroup was divided into four small groups as the phases of 2h., 6h, 12h, 24 h. The rats'duodenum were touched in group C. 5% sodium taurocholate was injected through pancreatic duct to made the SAP models in group P. After SAP Models were made, U-rokinase was injected in the rats of group T. The volume of blood flow, the level of TNF - a and ranked data of liver pathologic changes were compared among the third groups.Results1. liver blood flow; Comparing with group C, liver blood flow was decreased significantly( P <0.05) in group P and group T. Comparing with group T, liver blood flow was significantly decreased in group P. Difference among small groups in group T was not significant(P >0. 05).2. level of TNF - α: Comparing with group C, the level of TNF - a in all phases of group P and group T( P < 0. 05). Comparing with group T, the level of TNF - α in all phases is significantly higher(P <0. 05)3. Pathological changes: Comparing with group P, the liver tissues of group T have a lower level of pathological changes in all phases. The pathological changes have no significant difference in group T with group C.DiscussionIn the early stage of acute pancreatitis (2h) , Blood flow of liver decreased significantly, while pathological changes of liver tissues were seen. After the use of Urokinase, the liver blood flow and pathological changes of liver tissue were improved significantly comparing group P(P<0.05). It concluded that Urokinase can protect liver by improving it's rnicrocirculation. In our research, the level of TNF — α of group T decreased significantly comparing to group P, but higher than group C. It concluded that Urokinase has no direct effect to the level of TNF - α, but it can decrease the inflammatory reaction by improve microcir-culation.
