Objectives To explore the relationship between cognitive impairment and sleep disorders in patients with Mild Cognitive Impairment (MCI).To study the relationship between obstructive sleep apnea syndrome (OSAS) and cognitive impairment in the elderly. To observe the effect of melatonin on the sleep disorders and cognitive impairment in patients with MCI.Methods 74 MCI and 112 controls were tested by Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Mini-Mental Status Examination (MMSE), clock drawing test, word list recall and digital symbol test. 17 MCI and 29 controls were administered by polysomnography(PSG) in the home. Sleep studies were recorded in the subjects' homes and scored for sleep apneic events, and oxygen saturation levels.36 elderly people were random-sample at the beginning, and 16 of them were in the melatonin group, the others were in the controls .They were tested once more by PSQI, ESS, PSG , neuropsychoiogical tests ofMMSE, wechsler memory scale , clock drawing test,word list recall anddigital symbol test.Results The MCI subjects performed significantly lower in followingareas: age, time of education, time of memory complaints, MMSE, wordinstant recall, word delay recall, proportion of word delay recall, digitalsymbol test, lasting of sleep complaints, PSQI, ESS and sleep latency. (P< 0.05 ) . The time of memory complaints were significantly associatedwith time of sleep disorders (r=0.2544, P=0.0287 ) , age ( r=0.2589, P=0.0259) , ESS (r=0.2456, P=0.0349) .The results of digital symbol test were significantly associated with PSQI (r=-0.2391, P=0.0402) , age (r=-0.3363, P=0.0034) , time of education (r=0.4334, P=0.0001) .The results of MQ were significantly associated with latency period of sleep (r=0.65, P=0.0087) .The MCI subjects accompanied OSAS decreases in the sleep latency, increases in ESS (P < 0.05 ) , and the results of ESS were significantly associated with BMI ( r=0.8018 , P=0.0094 ) .The OSAS subjects decreases significantly in the delta sleep period and increases in the"apnea and hypopnea index, desaturations per hour (P < 0.05 ) . Performances on the MMSE, MQ, digital symbol test , clock drawing in the OSAS was impaired significantly compared with the control group. And the results of digital symbol test was significantly associated with MMSE (r=0.3821, P < 0.05 ) .The results of MMSE was significantly associated with age (r=-0.5332, P< 0.05) and mean saturation (r=0.4091, P< 0.05) The results of MQ was significantly associated with MMSE (0.726, P < 0.05) and sleep latency (r=0.4945, P < 0.05) and desaturation index (0.645, P < 0.05) .One, three, six months later, the results of PQSI> ESS in melatonin group were improved significantly (p < 0.05 ) .Six months later, the improving in melatonin group significantly in the following areas: sleep fficiency, sleep latency, MQ, MMSE, word instant recall, word delay recall, digital symbol test (P<0.05) .And after taking melatonin, such results...
