Clinical Study Of Mammographic Stereotactic Localized Biopsy On Nonpalpable Breast Lesions

Objective: To evaluate the value of stereotactic core needle biopsy (SCNB) or needle localized operation biopsy (NLOB) under mammography in the diagnosis of nonpalable breast lesion(NPBL). Methods: A prospective study is performed on the diagnosis of NPBL by using SCNB or NLOB and operating on the partial patients performed SCNB at the same time. By comparing and analyzing the pathological result of SCNB with operation' s, the manipulative course, the diagnostic accurate rate and the false positive rate as well as the false negative rate and so on, we arrive at the conclusion. Results : 29(23.58%) cases of breast cancer, 86 cases of benign lesions, 8 cases of normal tissue were found in the 123 cases , a total of 138 NPBL, of which SCNB of 67 cases, 82 NPBL and 56 cases of NLOB had been found .Abnormal mammographic imaging includes: 70 cases of microcalcifications, 18 cases of mass without microcalcifications, 25 cases of mass with microcalcifications, 5 cases of prickle , 11 cases of high-density, 9 cases of limitating structure-contortion . Of 67 cases of SCNB, 32 cases had undergone operation , comparing with the pathological result of SCNB with operation ' s , diagnostic accurate rate of SCNB was 84.38%, sensitivity rate 73.68%, peculiarity rate 100%,the false negative 26.32%, there were no false positives. In 56 case of NLOB, the accuracy rate of locating was 93.94% in the first time, lesion excising rate wholly was 89. 74% in the first time, there were no breast cancer cases during the recent follow-up visits. Conclusion: SCNB is a simple and less invasive method, however, with higher false negative rate , the application of SCNB should be very deliberative, it may be applied for patients of lower risk with breastcancer. NLOB was a golden norm for the diagnoses of NPBL, the diagnosis of NLOB was accurate ,but the manipulational course of NLOB was complicated, the patients of higher risk with breast cancer should select NLOB, because it removes the lesions completely, both diagnosis and treatment are executed at the same time, we do not worry about leading cancer transferring from? it. We believe that a lot of patients in stage T° breast cancer shall be found by selectivity applying of SCNB and NLOB, the long-time survival-rate of breast cancer shall be heightened accordingly.
