OBJECTIVE: The management of a series of 35 patients operated on for the cerebellopontine angle epidermoids is reviewed, to study the techniques and methods of the treatment for cholesteatoma with microneurosurgery METHODS: Clinical features ,CT and MRI characteristics of the patients were studied. The operative approaches,results and complications were also diccused . The suboccipital , infratemporal transtentorial approach and endoscope-assisted Microneurosurgery were used. RESULTS: Clinical features and surgical approaches varied according to location and growth pattern. A higher total removal rate was achieved in patients with tumors confined to the primary location. Total resection was achieved in 22 cases, little cyst-was remained in 9 cases and subtotal resection was made only in four,no mortality of all.Approximately half of the patients presented with transient mild focal deficit impairments resulting from the manipulation of the nervous structure over a wide area. There was a higher rate of surgical complications with mesencephalic extended cerebellopontine angle tumors. CONCLUSION: The suboccipital , infratemporal transtentorial approach are the main approaches for cyst resection. Total resection of the tumors could reduce the incidence of chemical meningitis.Endoscope-assisted microneurosurgery can increase the total-resection rate for tumors.
