Objective: With the improvement of people's living and medical conditions, people's life are becoming longer and longer. The change of modern medical model has much great effect on the people's health notion. Now people are not only pursue to longevity, but pay more attention to the quality of life. Activity of daily life is an important index to measure the health condition of the eldly. While the cognitive function is another necessary facet of health of the oldest people. The purpose of the study is to explore the situation and the affecting factors of the ability of daily life and the cognitive function. The risk factors of the life quality of the oldest could be found out in the study. Measures could be taken directly to improve the quality of life of the eldly. Methods: Five hundred and nineteen persons aged 90 and older were selected randomly among Hebei province. The abilities of daily life and the cognitive functions were investigated by using part of the scale of activities of daily life scale and the modified Mini-mental state examination scale. The oldest's common characters, habits, behavior ways and the prevalence rate of chronic disease were also investigated by the questionnaires. All statistical analysis were performed by statistical product and service solution(SPSS) software, Version 11.5. The prevalence rate of the abilities of daily life and the cognitive function were calculated according to criteria in China and other countries respectively. Influencing factors of the abilities of daily life and the cognitive function were analyzed by using simple Chi-square test and multiple Logistic regressions. The odds ratio(OR) and the 95% confidence interval of OR of each of these factors for the abilities daily life and the cognitive function were calculated. Results: The prevalence rate of the disabilities daily life of the people aged 90 and older of Hebei province was 27.36%. Multiple stepwise Logistic regression analysis showed that the difficulty rate of the abilities daily life in female was higher than that in male(OR=2.25, p<0.05). The difficulty rate of the abilities of daily life in those who have spouse was lower than those of bereft spouse(OR=0.58, p<0.05). The prevalence rate of the abilities of daily life in those who have good economy conditions was lower than those who have poor economy condition(sOR=0.69, p<0.05). Manual labour could prevent the decline of the abilities of daily life(OR=0.51, p<0.05). The better was the public opinion, the lower was the risk of having difficulty in the abilities of daily life(OR=0.46, p<0.05). It was showed that chronic disease could increase the risk of having difficulty in the abilities of daily life(OR=3.96, p<0.05). With the exception of cataracta glauca, the abilities of daily life wereaffected in variety degrees by all the diseases in the questionnaire(p<0.05). Multiple stepwise Logistic regression analysis showed that chronic bronchitis, cataract, Parkinson's disease, cerebrovascular disease and tumor were the main disease affecting the decline of the function in the oldest persons. The prevalence rate of the cognitive function of people aged 90 and older of Hebei province was 25.63%. Multiple stepwise Logistic regression analysis demonstrated that the difficulty rate of the cognitive function in female was higher than that in male(OR=2.90, p<0.05). The difficulty rate of the cognitive function in those who have spouse was lower than those of bereft spouse(OR=0.34, p<0.05). The study also showed that regular physical exercise could prevent the decline of the cognitive function(OR=0.59, p<0.05). The results of the study indicated that the smokers had a higher risk of having difficulty in the cognitive function compared to non-smokers(OR=0.58, p<0.05). Those who have the difficulty in the abilities of daily life had a higher risk of having difficulty in the cognitive function than those who have the normal abilities of daily life(OR=2.19, p<0.05).
