Cord Blood IgE Level Correlation Of Development Of Atopic Diseases In Infants

Objective: To determine whether cord blood IgE predicts the development of asthma and other IgE-mediated allergic diseases in infants at 18 months or 6 months.Methods: Total cord blood IgE level of 334 infants were measured using Fluoroenzymeimmunoassay UNICAP100R (Pharmacia & Upjehn). 304(91.0%) of the infants were followed up by telephone or were asked to come to clinic. The atopic diseases such as dermatitis, recurrent wheezing, urticaria and others were recorded carefully. All data were analyzed statistically to use SPSS 11.0 for Windows.Results: Four infants with cord blood IgE greater than 10KU/L were excluded from the analysis because of possible contamination with maternal blood. 30.4%(100/229) infentshad detectable total IgE in cord blood 0.35KU/L.88(28.9%)of the 304 infants followed up had the detectable IgE . 54(61.4%) of the 88 infants had the allergic diseases , 41(41.6%) had the dermatitis, 18(20.5%) had the urticaria and 5(5.7%) had the recurrent wheezing .The corresponding figures of the infants with cord blood IgE <0.35KU/L were 77(35.6%), 62(28.7%), 20(9.3%) and 10(4.6%), respectively. There was significant difference between two groups in the development of the allergic diseases, of the dermatitis and of the urticaria(P=0.000, 0.003, 0.007, respectively). Multiple stepwise regression analysis showed there was significant correlation between the cord blood IgE 0.35KU/L and the development of the allergic disease,of the dermatitis and of the urticaria of the infants. There was no correlation between the cord blood IgE and the recurrent wheezing of the infants(t=4.442,P=0.000;t=3.122,P=0.002;t-2.538,P=0.012;respectively). 77(25.3%,77/304)infants had family history of the atopic disease .43(55.8%) of the infants with family history had the allergic disease, 32( 41.6%) had the dermatitis, 7(9.1%) had the recurrent wheezing and 13(16.9%) had the urticaria. The corresponding figures of the infants with no family history were 87(38.3%), 71(31.3%), 8(3.5%) and 25(11.0%), respectively. There was significant difference between two groups in the development of the allergic diseases and the recurrent wheezing (P=0.007 and P=0.051). Multiple stepwise regression analysis showed there was significant correlation between father's atopic history and the urticaria of the infants (t=3.800, P=0.000). The mother's history was related to dermatitis of the infants (t=2.872, P=0.005). The parents history was related to recurrent wheezing (t=3.813, P=0.000). The infants born in the autumn had highly association with the dermatitis of the infants. Boys had higher rate of the dermatitis and the recurrent wheezing than girls. Milk and the breast feeding 3 months had highly association with the recurrent wheezing of the infants. The positive predictive values(PPV) of the cord blood IgE 0.35KU/L were 61.4%, the specificity 80.5%,the sensitivity 41.5%,the relative risk(RR) 1.74. The PPV of the family history was 55.8%, the specificity 80.5%,the sensitivity 33.1%,the RR 1.46.The PPV of the cord blood IgE 0.35KU/L and/or the family history was 57.7%, the specificity 65.5%, the sensitivity64.5%, the RR 1.96.When the cord blood IgE 0.35KU/L gathered with the family history the PPV was 65.2%, the specificity 95.4%,the sensitivity 11.5%.The PPV was higher(P>0.05), the sensitivity was lower (p<0.01) ,the specificity was highest (p<0.01).Conclusion: The atopic disease especially the dermatitis and urticaria of the infants was significantly more frequent among infants with the cord blood IgE 0.35KU/L. The elevated cord blood IgE was found a significant predictor for the development of the atopic disease especially the dermatitis and urticaria of the infants. The PPV was highest when the cord blood IgE 0.35KU/L combined with the family history .
