Objective: To observe the clinical effect of Ruanmaijiangzhi capsule (RMJC) on treating dyslipidemia and protecting vascular endothelium and explore its therapeutic mechanism. Methods: Seventy-two cases of patients with dyslipidemia were divided randomly into three groups: 24 cases in group A (RMJC), 24 cases in group B (Lovastatin Tablets), 24 cases in group C (Xuezhikang Capsule). The changes of symptoms and laboratory indexes in all the patients were observed before and after treatment, the laboratory indexes including blood lipid, anti-oxidation, secretion function of vascular endothelium and endothelium-independent relaxing function of brachial artery. Sixty rats were divided randomly into blank group, model group and group ABC, twelve rats in every group. Excluding the blank group with common animal feed, the other groups were fed with high fat diet in order to result in dyslipidemia models. Then group ABC were respectively given RMJC, Lovastatin and Xuezhikang additionally. Laboratory indexes in all the rats were determined. Results: After fifty days of treatment, clinical symptoms of group A were effectively improved. The total effective rate of relieving symptoms was 95.83%, which was superior to those of and C. The serum total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL) decreased. The total effective rate of reducing lipids was 91.67%, not differing from that of group B and C. And the effect of reducing TC was worse than that of group B but same as group C, the effect of reducing TG was same as group B but better than group C.Meanwhile, the endothelium-independent relaxing function of brachial artery of the patients was improved obviously and the nitric oxide (NO) levels rose markedly, the endothelin (ET), ET/NO, ET/calcitonin gene-related peptide (ET/CGRP), and lipoprotein [Lp(a)] levels lowed, group A had similar effects with group B and group C. The elevation of super oxide dismutase (SOD) and the reduction of malonyldialdehyde (MDA) in group A was superior to those of the two control group. Animal experiment showed that RMJC had similar effects of lowering lipid, resisting lipid peroxidation and regulating secretion of vascular active substances. Conclusions: RMJC had affirmative effect of reducing blood lipid levels, could effectively relieve clinical symptoms and protect vascular endothelium. The therapeutic mechanism of protecting vascular endothelium might be associated with modulating lipid metabolism and resisting lipid peroxidation.
