[objective] To study the insulin resistance assessed by the reduced sample number of minimal model method , and discuss the change of serum free fatty acids (FFA) and C-reactive protein(CRP) in the fasting state, and the relationship between FFA , CRP and the insulin resistance in the patients with obesity and impaired glucose tolerance(IGT).[Methods] Insulin sensitivity index(SI),glucose effectiveness(SG) and acute insulin reactive to glucose(AIRg) were measured by the minimal model method with reduced frequently sampled intravenous glucose tolerance test (FSIGTT)in patients with obesity,IGT and in the controls;The serum FFA and CRP levels in the fasting state were measured by calorimetric and animmunological turbidimetry test in patients with obesity and IGT as well as in the controls; Body mass index(BMI) and waist hip ratio(WHR) were calculated in three groups.[Results] SI was significantly higher in the controls than those in the obesity and IGT patients( p<0.01); More serum glucose and insulin levels acquired from the FSIGTT in obesity and IGT groups were higher than those in the controls(p<0.05); AIRg in the controls and IGT groups were significantly lower than that in the obesity goup( p<0.01),but there was no markedly difference between the controls group and IGT group; Among all the subjects of the study,SI was negatively associated with WHR( p=0.000) and BMI( p=0.002). The fasting serum FFA and inslulin levels in patients with obesity and IGT were higher than those in the controls;FBG and HbAlC in the IGT group were higher than those in the obesity and control groups(p<0.05).Correlation analysis showed that fasting FFA level was negatively correlated with SI (r=-0.658,p<0.01) .positively with BMI (r=0.448,p=0.017) andWHR(r=0.599,p=0.001). Mutiple regression analysis showed that the FFA level in the fasting state was positively correlated with HbAlC(t=5.191,p=0.000) and WHR(t=4.657,p=0.000);The fasting serum CRP levels in patients with obesity and IGT were higher than those in the controls;Correlation analysis showed that fasting CRP level was negatively correlated with SI (r=-0.603,p=0.001) positively with BMI (r=0.666,p=0.000) and WHR (r=0.390,p=0.040) .[Conclusions] (1) The minimal model method of reduced sample number is a reliable method to estimate insulin resistance. (2)The fasting serum FFA levels were elevated in the patients with obesity and IGT. The fasting serum FFA levels were negatively correlated with SI, and positively with fasting serum glucose , BMI, WHR and HA 1 Complying the association with insulin resistance . (3) The fasting serum CRP levels were higher in the patients with obesity and IGT than those in the normal,and CRP were negatively correlated with SI, positively with BMI and WHR,showing a strong association with insulin resistance.
