The Changes Of SICAM-1 And IL-8 Concentrations In Serum Of Neonates With Hypoxic Ischemic Encephalopathy

Objective To study the changes of the soluble intercelluar adhesion molecule-1 (sICAM-1) and the interleukkin-8 (IL-8) in the serum of neonates suffering from hypoxic ischemic encephalopathy (HIE) , and the significance of the change of the serum sICAM-1 and DL-8 in HIE pathogenesis.Methods According to the state of illness, 36 cases were divided into three groups, which were mild group (13 cases), moderate group (14 cases) and sever group (9 cases). The control group is 17 normal neonates. The sICAM-1 and IL-8 concentrations in the serum were detected with enzyme-linked immunosorbent assay (ELISA) at the critical stage and the beginning of convalescent stage in 36 cases of HIE neonates and 17 normal neonates. The data are analyzed with analysis of variance, Newman-Keuls q test, Dunnett t test and linear correlation.Results (1) The concentrations of sICAM-1[(216.64 85.32)pg/ml] and IL-8[(59.85 30.45)pg/ml] at the critical stage of the 36 cases HIE neonates were significantly higher than those [(6.16 4.05)pg/ml, (6.13 2.25)pg/ml]of the control group(q=17.4137, 11.8814, P<0.001) . Both the concentrations of sICAM-1, [(78.44 31.68)pg/ml] and IL-8 [(21.65 14.01)pg/ml] at the beginning of convalescent stage were significantly lower than those at the critical stage(q= 14.273 5 , 10.4400, P<0.001 ).But they were significantly higher than the control group's (q=5.9814, 3.5195, P<0.001, 0.05).(2)The sICAM-1 concentrations in the serum at critical stage of mild, moderate and sever group were (158.29 77.09)pg/ml, (217.03 45.61)pg/ml and (300.32 78.39)pg/ml. The differences of three groups were significant(q=8.4348, 5.0203, 3.9273, P<0.001 , 0.01). The serum sICAM-1 concentrations at the beginning of convalescent stage of mild, moderate and sever group were (58.04 26.69) pg/ml, (86.40 29.00)pg/ml and (95.57 28.98)pg/ml. The difference between moderate and sever group was not significant (q=1.3069 P>0.05). But the differences between mildand moderate or sever group were all significant (q=5.2669, 4.4800, P<0.01). The sICAM-1 concentrations in the serum of three groups at the beginning of convalescent stage were all lower than those at the critical stage. (t=7.3498 , 9.0498 , 4.4305, P<0.001), but higher than that of control group(q=8.5691, 13.5291, 13.1991, P<0.01). (3) The IL-8 concentrations in the serum at critical stage of mild ,moderate and sever group were (38.98 19.83)pg/ml, (59.77 25.47) pg/mland (84.54 30.23)pg/ml. The differences of three groups were significant (q=7.4862, 4.2516, 3.7120, P<0.01 , <0.05). The IL-8 concentrations in the serum at the beginning of convalescent stage of mild, moderate and sever group were (13.00 4.07) pg/mk (24.62 11.60) pg/ml and (29.52 20.22 ) pg/ml .The difference between moderate and sever group were not significant (q=1.1563, P>0.05). But the differences between mild and moderate or sever group were all significant (q=5.1812, 4.0968, P<0.01). The IL-8 concentrations in the serum of three groups at the beginning of convalescent stage were lower than those at the critical stage(t=4.9132,4.6263,4.5390,P<0.001). The differences in serum IL-8 level at the beginning of convalescent stage were significant between the moderate or sever and the control group(q=7.7100, 6.9535, P<0.01), no significant between the mild and the control group (q=2.5285, P>0.05). (4) The concentration of sICAM-1 in the serum is positively correlative to the concentration of the IL-8 in the serum of 36 neonates with HIE (r=0.843,PO.001). There are also positively correlative between the concentration of sICAM-1 and IL-8 in mild, moderate and sever group respectively(r=0.712, 0.656, 0.896, P<0.01, 0.001).Conclusion The sICAM-1 and IL-8 play a role in the brain damage of HIE pathological process. The sICAM-1 and IL-8 concentrations rise sharply at the critical stage of HIE. They are closely related to degree of brain damage.
