Abjective We detected the quantity and ability of CD4+T and CD8+ T cells in peripheral blood and liver tissues ,and investigated the change of cytokines in the liver to study the probably mechanism about HBV affect cellular immunity system in human body. Method (1)The quantity of CD4+T and CD8+ T cells in peripheral blood was detected by flow cytometry(FCM) in 36 patients with hepatitis B and 20 cases of controls;2)The expression of HBsAg and HBcAg were detected by streptavidin-peroxidase immunohistochemistry(SP) in 64 liver tissues which come from 10 cases of patients with chronic hepatitis B(CHB) , 37 cases of liver cirrhosis( LC)and 17 cases of hepatic cellular cancer(HCC),14 cases of HCC with paratumor cirrhosis(PC) .Take 50 cases of patiets that HBsAg and(or)HBcAg were positive as group of HBV (+),and 10 cases of patiets that HBsAg and HBcAg were negative in liver tissues and blood serum as group of HBV (-). Then detect and compare the differentiation of the expression of CD4, CD8, CD25, TNF-αand TGF-β2 in liver tissues of the two groups . Results(1)The quantity of CD4+ T cells and the ratio of CD4+T/CD8+T in peripheral blood of patients was significantly lower than controls;the quantity of CD8+T cells was considerably higher than controls. (2)T lymphoid cell infiltrates in the liver were most gathered in the lobules and the portal tract .The densities of CD8+T cells were higher in lobules and portal tracts than CD4+T cells. The quantity of CD8+T and CD4+T cells in the liver tissues of HBV positive group have significant difference with the liver tissues of HBV negative group(Pb﹤0.05). The quantity of CD25+T which relative to ability of immunity in HBV positive group was less than HBV negative group, but it have no significant difference between two groups.(3)The expression of cytokine TNF-αin the liver tissues of HBV positive group have significant difference with the liver tissues of HBV negative group(Pb﹤0.05).But the expression of TGF-β2 in the lliver of HBV positive group have no significant difference with HBV negative group (Pa﹥0.05).TGF-β2 and TNF-αwere expressed in liver cirrhosis,HCC and paratumor cirrhosis,have significant difference with control group(Pc﹤0.01). [Conclusion](1)The T lymphoid subsets of peripheral blood mononuclear cells and the liver tissues of patiets infected with HBV was disordered,the activity of T cells decreased and the ability of immunity was weaken possiblly.(2) The expression of cytokine TNF-αhave relation to the infection of HBV and lesions of liver, but the infection of HBV seem have no relation to the expression of TGF-β2 .During the occurrence and development of liver cirrhosis and HCC, TGF-β2 and TNF-α is two important cytokines implicated as important pathogenic mediators in the development of liver fibrosis and HCC.
