Object To study the pathogenesis of PLA2 activation in severe acute pancreatitis (SAP) and its role to distant organ-small intestine in the rat. To investigate the effects of a PLA2 inhibitor phosphate chloroquine (PC) as pre-treatment and past-treatment in SAP.Methods 42 wistar rats were divided randomly into seven groups: Group A (Control, n=6), Group B (AP 6 hours, n=6), Group C (AP 6 hours+ PC-pre-injection, n=6), Group D (AP 6 hours+PC-past-injection, n=6),Group E (AP 12 hours, n=6), Group F(AP 12 hours+PC-pre-injection, n=6) ,Group G(AP 12 hours+PC-past-injection, n=6).The severe acute pancreatitis model was established by retrograde injection of 5% sodium taurocholate (STC) into the pancreatic duct. For studies, PC (30mg/kg) was administered intraperitoneally 1/2 hour before the STC injection in Group C and F. PC (30mg/kg) was administered intraperitoneally 1 hour after model was established in Group D and G. Examine the PLA2 activity in the serum and small intestine, and other various items of the rat.Results The PLA2 activity in the serum and small intestine in Group B and E is significantly higher than that in Group A (P<0.05). There are significantly positive correlate between the level of PLA2 activity and the amylase (AMS), TNF- a in serum and the MDA in small intestine. The PLA2activity in the serum and intestine reduced accordingly by the pre-treament and past-treatment with PC. The amylase and TNF- a in serum , the MDA in small intestine in Group C,D and group F,G was significantly lower than that in Group B, E(p<0.05 ). The score of the pancreas deduced accordingly. There are no differences between the pre-treatment group and past-treatment group (p>0.05 ) . There are no differences between the 6 hour group and the according 12 hour group (p>0.05).Conclusions The activation of PLA2 exists and is higher in the early stage in SAP than normal, and plays an important role in the subsequent injury. The pre-treatment and past-treatment with PLA2 inhibitor- PC can effectively reduce the level of PLA2 and alleviate the harm level of the pancreas and the distant organ-small intestine by many paths, then improve the prognosis of acute pancreatitis.
