At present, in the reform of the medical care insurance system in our country, we have preliminarily developed urban employees' basic medical care insurance system combined with overall social medical care fund and individual medical care account. In 1999, the catalogue of national basic medical care insurance medicine (Catalogue for short) was laid down and issued to further reinforce the administration of basic medical care insurance medication. With the specification of medicine in terms of the Catalogue, the medical care has been guaranteed and promoted. In the past few years, medication problem has been a social-focused problem for that material medical takes major part of medication expensive and the phenomena of big prescription, backhander of drugs become more and more popular. It seems more important to do evaluating research on hospitalizing medicine for patients with medical care (employees who take part in urban basic medical care insurance) or non medical care (those with public medical care or at their own expenses).In this essay, three comprehensive hospitals in Xi'anresponsible for medical care were selected to conduct survey and its analyses. We want find out the real reasons of medication problem in different kinds of hospital from the analysis. The reasonable suggestion of medication policy will be put forward ground on this research and the administration of medication will become more sound and scientific in the future. The study data included the front pages of 58091 resident medical records of three hospitals from July to December in 01, 02 and 03 and detail data of hospitalizing medicine. And the raw data were pretreated as following. After processed, 48807effective medical records were obtained.The research was based on the analysis and compare of urban employees' basic medical care insurance system and the catalogue of national basic medical care insurance medicine. To evaluating the condition of medical insurance reform and elucidating the problem of medication using, the structure, variety and effect factors of hospitalizing expense for patients with or without medical care in Xi'an were analyzed in this study.Methods: 1. Data were picked up from Military Medicine One; 2. All data collected were stored in Excel format; 3. Medicines of medical care patients were compared and identified in terms of the medicine catalogue; 4. Statistical analysis was conducted by Excel; 5.The analysis of expenses was shown as average.After the analyses of structure, variety and effect factors of hospitalizing expense for patients with or without medical care, we found: 1. The medicine expenses and the percentage of medicine expenses of non medical care patients of the hospitalizing expenses increased every year; it was relatively rational that medicineexpenses of medical care patients accounted for 31% of hospitalizing expenses; medicine expenses of non medical care patients increased fast while those of medical care patients increased slowly, which indicated the medical care policy effectively controlled their over-consumption of medicines. 2. The real reason affecting over-growth of medicine expenses was the increased amount of medicine and the promoted grade of medication. 3. The expenses of antibiotics were rated first in the expenses of other medicines, and in the administration of antibiotics too many kinds of high-grade medicines were used while too few low-grade, and the consumption level was too high.After the analyses of medication of medical care patients within or -out of the Catalogue, we found: 1. The self-paid medicines accounted for too great in the whole medicine expenses; 2. Relatively more low-grade medicines were chosen from the Catalogue for hospitalizing patients of medical care, while fewer high-grade medicines from the Catalogue; the too much higher percentage of self-paid medicines were related to the imperfect Catalogue laid down.After the comparison of medicine expenses and medicine grades of three hospitals beyond the Cat...
