China is a country with the highest number of old people in the world and with highest accelerating aging rate too. It was predicted that those old people aged above 60 will make up 12.6% of whole population in the country in 2010. However the prolonging of expected life-span doesn't mean the healthy aging, currently the health status of old people in our country is not satisfying, even becoming worse. Among the most common diseases which old people have are as follows in sequence: hypertension, coronary heart disease, diabetes mellitus and stroke, besides, dementia, malignant tumor, lung infection, osteoporosis, and prostate hyperplasia have relatively high occurring rates, mortality and disability rates.In the year of 2000, made a survey and obtained the basic data on the health status and medical service requirements for the 1811 people in parts of Xi'an aged people community. In 2003 we randomly took 596 cases out and made a follow-up 3-year visit, by comparing and analysis between the two times survey result, we could detect the dynamic changes and the affecting factors of the diseases and health status. The study will provide the basis for probing a better and realistic local model for health service and protecting way, meeting the great demands from the old people, improving their life qualities; at the sametime, it'll provide useful information on diseases prevention and dynamic condition of health status for Xi'an old people, esp. for the evaluation of efficiency of community service.Purpose: (1) Observing the characters and affected factors for the diseases and health status of old people in Xi'an parts of community in longitudinal fashion, providing basis for community setup; (2) Investigating the risk factors of hypertension and searching preventing ways.Methods: (1) Clustering the sample cases from parts of community people, 596 over 63 years old people, making 2 times visit during 3 year period, making analyses on the same group people in the year of 2000 and 2003 with three factors, including chronic disease rates, type of disease, healthy behaviors, by way of answering sheet and physical examination obtaining the data , making analysis by use of SPSS software; (2) On the basis of accumulating data, making a group case-control study for the 206 hypertension patients, taking 206 cases for disease group and non-hypertension patients 361 cases for control group, making a Logistic Rregressing Analysis based on the mono and multiple variations.Results: (1) Whole morbidity rate of old people is 76.0%, the rate with one disease is 29.4%, with 2 diseases is 22.8%, with more than 3 diseases is 23.8%, hypertension rate is 36.3%, the highest, the next one is arthritis with 17.6%; there is significant difference between groups with different professional and education background (P<0.05); (2) Mono-variation analysis indicated that the factors related with hypertension are: profession, education level, body mass index (BMI), nitrogen intake, family history, frequency of physical exercises. Multiple-variation Logistic Regressing Analysis suggested that independent risk factors of hypertension included: hypertension family history (OR=6.1) BMI (BMI > 24, OR=1.4), profession (OR=1.3). While physical exercise (OR=0.6) is a positive preventive factor; (3) There were obvious increases of chronic diseaserates in 2003, esp. the arthritis, cataract, cervical spinal illness and cerebral vessel disease, P value is < 0.01. There were obvious increases of type of disease in 2003 comparing with 2000, P value is < 0.01. Furthermore, the health behavior was becoming worse in 2003 comparing with 2000, the number of people who paid attention with physical exercise, physical examination, receiving health education, psycho consulting and preventive drug intake was reducing, only exception was the declining of number of people with overdose drink, P value is <0.01.Conclusion: (1) There's an obvious increasing trend for the higher chronic diseases rate with age become old.
