Background and purposeFatty liver disease (FLD) is a disease with steatosis in liver which can been caused by many reasons. Recently, the incidence of FLD is increasing along with the upgrade of living condition, remodelling of food structure and the application of new detective technique. The suffering age is lower than before on the whole.The main etiology of FLD are excessive alcohol intaking, obesity, hypertriglycerisemia, malnutrition, insulin resis-tance, toxicosis of drug, diabetes mellitus, viral hepatitis and hereditary genie abnormality of lipase et al . However, there are Approximately 20 percent cases without known etiology. As far as molecular pathology is concerned, lipid and lipoprotein metabolic disturbulence, lipidperoxidation, Oxygen irritability, immunoreaction et al are all involved in the responsibility.Epidemiology research indicate that FLD and typ II diabetes mellitus are often coexistence in the same patient .We name this FLD case Diabetic Fatty Liver Disease (DFL) .As some report, the incidence of DFL in Type II Diabetes Mellitus patients is approximately 21%-78%. We have known that Insulin Resistance and the Lipid Disturbulence result from the former are important pathological events in a patients with Type II Diabetes Mellitus. And we can say for sure that lipid diturbulence is an important etiology of Fatty Liver Disease.As the second most influential liver disease in the nation, FLD isresearched more and more. But the treatment of FLD now is still in confusion. There are many drugs used in practise , however , the curative effects are uncertain. For this phenomenon , the most important reason is that we have not enough knowledge of the etiology of FLD in detail. So much as some physicians treat patients suffered from FLD by antiatheroscl-erotic drugs unchangeably. So it is important to explore the etiology of FLD in detail to meet with the need of practise. In this research, the author try to explore the etiology of Diabetic Fatty Liver Disease(DFL) by retrospective analyse of clinical data from Type II Diabetes Mellitus patients with (or without) Fatty liver Disease.Material and MethodsMaterialClinical data of 215 patients in hospital, 2002-2003, with Type II Diabetes Mellitus were retrospectively analysed. All the cases were diagnosed by the Diagnostic Criterion of Diabetes Mellitus maked by the consultant commission of WHO,1998. Of all the cases involved in the search , 110 were male whose mean age was 56. 29; 105 were feraal whose mean age was 59. 99. All cases had not been treated by insulin injection before and cases with excessive alcohol intaking, hypothyroidism, chronic glomeru-lonephritis, nephrotic syndrome had been excluded out of the research. Fatty liver disease was diagnosed by brightness mode ultrasound scan. The diagnostic criterion of FLD is listed as follow:(1)Resonance of hepatic tissue is increased (2) Resonance of hepatic depth is decreased (3) Blood vessels of the liver are displayed faintly. At last, the research group was made up of type II diabetes mellitus patients with FLD(DFL) and the contrast group was made up of type II diabetes mellitus patients without FLD.MethodsAll cases were experienced Glucose Tolerance Test universally. FPG(fasting blood glucose), 2HPG(blood glucose measured in two hours after taking food), The concentration of Triglyceride(TG), Total Cholesterol (TC),Very Low Density Lipoprotein (VLDL) were measured by biochemical analysis at the same time. The concentration of Fins (Fasting Blood Insulin) and 2hlns(blood insulin measured in two houres after taking food) were measured by radioimmunoassay. Homeostasis model was established to assess the IR (index of insulin resistance) and the IS (index of insulin sensitivity to assess the capability of pancreas islet B cell).
