Objective: To investigate the factors of long-term survival of small primary liver cancer after surgical treatment Methods: Two hundred and fifty-nine cases of liver cancer that received surgical treatment in 13 years were analyzed retrospectively. Life-stable and Cox proportional hazards model were used for analysis of prognostic factors.Results: Comparisons between the small liver cancer (<3cm) and the greater one (3~5cm) revealed that the small liver cancer had a higher survival rates than the greater one (1 year 89.13 ± 2.88% versus 66.59 ± 4.86%;3 year 77.76± 4.79% versus 55.75± 6.12%;5 year 67.39 ± 6.88% versus38.15 ± 9.39%). And there were same results between the tumor with capsule and the one without capsule, between single-nodule and two nodules, and between regular hepatoectomy and irregular hepatoectomy. But there were similar survival rate in the groups with or without Pringle's maneuver, HBsAg(+) or HBsAg (-), AFP(+) or AFP(-), child A or B, and HCCor non HCC.Conclusion: Resection is still the first choice for treatment of small primary liver cancers. Small liver cancer single-nonude capsule and minor resection were important factors to prolong survival further. There were not enough evidences to show that Pringle's maneuver, HBsAg, AFP, child grade or the sort of pathology could influent the prognosis of the small PLC.
