Influences Of Naoyian On Plasma ET And NO Level In Patients With Intracerebral Hem-orrhage Of Wind-syndrome Caused By Sthenic Liver-Yang

Intracerebral Hemorrhage(ICH) is a most devastating type of acute cerebrovascular diseases with a high rate of incidence,morbidity and mortality. Up to now there was no important breakthrough on the treatment of ICH,and it is count for much to look for the valid treatment of medicine and study the mechanisms of its function. By analysing traditional Chinese medicine literature and by combining modern research results, according to the characteristics of the occurrence of ICH, this paper considers that wind,fire,phlegm and stagnant blood are the basic pathogenesis of this disease,and subduing the liver to stop wind,cooling blood and purging fire,activating blood and resolving phlegm are the basic treatment methods to ICH. Nao Yi an granule(NYA) which can subdue the liver to stop wind,cool blood and purge fire,activate blood and resolve phlegm ,is one of the 3 categoyies of new medicines of the Chinese herbalMedicine.and a series of clinical and experimental stndies has becn proceeded to prove its curative effect and functional mechanisms.In order to further staby its clinical curative effect, 65 cases with acute ICH of Liver Yang into Wind syndrome were randomly divided into NYA group and western medicine(WM) group according to the order of the patientswho are receiving treatment in the hospital. The patients in NYA group were treated with NYA pills,while the patients in WM group were treated with normal western medicine. The curative effect was observed after 4 week treatment,and the result shows: the recovery rate and obviously curative effective rate of NYA group were 80.6%, the total effective rate of NYA group was 93.5%, the hematoma entire obsorptivity of NYA group was 53.3% by CT scan, and the ordinalrates of WM group were 61.8%,86.8% and 37.5%.The effects of NYA are better than those of NO(p<0.05,p<0.05,p<0.01).Endothelin(ET)and Nitric Oxide (NO)are shrinking andexpanding genes from the endodermis discovered in recent years, they form a balance system together and participate in the regulation of the cerebrovascular strain, their action in the pathogenesis of ICH has already caused recognition. In order to further discuss the efficacy mechanisms of NYA on ICH, before treatment and after 4 week treatment the elbow venous blood of the ICH patients in the two groups were drew in the morning before their breakfast in this study, the contents of ET and NO in acute stage and 4 weeks after treatmentwere examined,and the result shows: plasma ET levels increased and NO levels decreased significantly in acute stage of ICH; plasma ET levels decreased and NO levels increased significantly in the NYA group after 4 week treatment, almost returned to the normal levels, and there was remarkable difference between the NYA group and the WM group. From the resnlts above, NYA can promote the nerve function resumed and mend the hematoma absorbed which has further prored its clinical curative effects that NYA can regulate plasma ET and NO levels, and improve the cerebrovascular function, which may be one of the protective mechanisms of NYA from ischemic injury after ICH.
