The preservation of the harvested organ constitutes a prerequisite for organ transplantation. For kidney preservation, hypothermia storage remains the most common technique in use. Although UW solution is regarded as the Golden Solution because of its better outcome in clinic and experimental transplantations, it is not widely used in renal transplantation in our country due to its high expense,, HC-A solution is the most widely used internal solution for kidney preservation , its value has been testified by its practices over 20 years. Nevertheless, the delayed graft function(DGF) related to acute tubular necrosis (ATN) still remains an important complication after transplantation .Whichever used, UW solution or HC-A solution, the incidence of DGF is not small. The incidence of ATN following renal transplantation is often quite high, occurring in 10 to 40% of the recipients of cadaveric donor kidneys, the improvement of quality preservation and the development of new preservation strategies could be an important therapeutic challenge.Recent studies have demonstrated that ischemia and reperfusion contribute to the nonimmunological damage that complicates transplantation. The use of PEG in a preservation solution has been associated with a decreased incidence of rejection in clinical and experimental heart transplantation. In addition, the mode of action includes the prevention of osmotic swelling . It has been reported that solution with PEG 20000 may release ischemia and reperfusion injury. The aim of this study is to amend the HC-A solution which has been used for 20 years, enhancing the buffer capacity and strengthening the ability aginist ischemia and reperfusion injury.then assess the effect of new HC-A solution with PEG (molecular weight 20,000) as an impermeant material on hypothermic swelling as well as a protectionaganist lipid peroxidation during hypothermal preservation of kidney. and histology changes, compared with HC-A solution and UW solutions,,Methods:15 SD rats was divided into three groups randomly, perfused and stored at 0-4 with amending HC-A solution containing PEG20000 (PHC-A), amending HC-A solution and UW solution respectively, sampled at ON 12 24, 48 hours of storing time, then determined water content and Malondialdehyde (MDA) level in renal tissue, examined the histological changes after 48 hours Results:Tissue water content with PHC-A solution was lowerer than that with amending HC-A solution (p <0. 01) and UW solution (p<0. 05); MDA tissue levels with PHC-A solution was lowerer than that with amending HC-A solution (p<0. 01) and UW solution (p<0. 05); Histological Analysis Grade of PHC-A solution was superior to HC-A solution (p<0. 01) and UW solution (p<0. 05)Conclusion:amending HC-A solution containing PEG 20000 present better protection to the hypothermal preservation of the isolated perfused rat kidney ?Such outcome demand the vertification of experimental study of transplantation.
