Objective Hptertension can cause the change of left ventricular geometry. The prognosis of hypertensive patients with different geometries of left ventricle is different. Therefore, the changes of geometry are emphysized recently. However ,the traditional 2-DE echocardiography can only display the general construction and function of heart with different geometries by detecting the dimension of cavity,the thickness ,echo and motion of wall ,and can not ascertain the pathological changes of tissues ,which can induce the subjectivity in clinical diagnosis .Ultrasonic tissue characterazation(UTC) can assess pathological changes of myocardial tissue based on detecting and quantitating the acoustic properties of myocardium .UTC consists of identification and characterazation of the abnormalities in the physical or physiological state of biological structures based on analyzing interactions between ultrasound and tissues .Color Kinesis(CK) is a new echocardiographic technique that uses color-encoding to depict systolic or diastolic endocardial motion .which allows objective evaluation of regional wall motion .This study is to detect the parameters of ultrasonic integrated backscatter and color kinesis hi essential hypertensive patients .The aim of this study is to determine the feasibility of the use of integrated backscatter and color kinesis to assess the degree of myocardial injury and regional wall motion hi essential hypertensive patients with different left ventricular geometries.methods 72 cases of essential hypertensive patients were divided into four groups based on LVMI and RWT which can be calculated from echocardiographic data with HP5500 ultrasound machinemonnal geometry, concentric remodeling, concentric hypertrophy, eccentric hypertrophy groups .The parameters of myocardial ultrasonic integrated backscatter and regional wall motion were measured in 72 cases of differentgeometries and 31 normal cases as control.Results rVS-IB% increased in all hypertension groups , concentric and eccentric hypertrophy groups had significant difference with the rest; In concentric and eccentric hypertrophy groups, IVS-CVIB decreased and LVPW-IB% increased, LVPW-CVIB decreased in eccentric hypertrophy group; Among all ultrasonic integrated backscatter parameters, eccentric hypertrophy group had significant difference with the others; Transmural gradient in myocardial ultrasonic integrated backscatter were not present in hypertension patients . SEM of IVS and LVPW decreased in eccentric hypertrophy group ,but there is an increased tendency in concentric hypertrophy group . In eccentric hypertrophy group ,the color band is thin or appear ,and the color stratification is not integrant in end-systolic ck images.Conclusions Ultrasonic backscatter parameters are valuable in determination of the degree of myocardial injury of essential hypertensive patients with different left ventricular geometries. IVS-IB% increased in all hypertension groups , concentric and eccentric hypertrophy groups had significant difference with the rest; LVPW-IB% increased in concentric and eccentric hypertrophy groups; CVIB decreased in concentric and eccentric hypertrophy groups; Transmural gradient in myocardial ultrasonic integr -ated backscatter were not present in hypertension patients .Color kinesis parameters are useful in the assessment of the regional wall motion of essential hypertensive patients with different left ventricular geometries.SEM of IVS and LVPW decreased in eccentric hypertrophy group,but there is an increased tendency in concentric hyper -trophy group.In eccentric hypertrophy group ,the color band is thin or appear.and the color stratification is not integrant in end-systolic ck images.
