0bjective:To assess the severity of myocardial damage in patients with primary hypertension(PH) by ultrasonic integrated backscatter.Methods:The total 95 cases of study subjects including 67 patients with PH which were all diagnosised by clinic and 28 cases of nomal subjects were enrolled in this study. They were divided into three groups, the non-left ventricular hypertrophy(NLVH) group,left ventricular hypertrophy(LVH) group and the normal group.In the NLVH group,there were 17 cases of male and 13 cases of female age range (56.4±8.6 )years, average age 56.3 years old, body surface area(BSA) (1.65±0.21)m2, left ventricular mass index(LVMI) lower than 125 g/m2。In the LVH group,there were 20 cases of male and 17cases of female age range (58.2±9.2) years, average age 56.3 years old, body surface area(BSA) (1.70±0.25)m2, LVMI higher than 125 g/m2。In the normal group,there were 15 cases of male and 13 cases of female age range (54.6±7.9) years, average age 52.9 years old, BSA (1.66±0.20m2)。The thickness of interventricu1ar septum and 1eft ventricularposterior wall at end diastolic(IVSd and LVPWd),left ventricular mass(LVM) and left ventricular mass index(LVMI)in the three groups were measured by M-mode and Two-dimensional echocardiography respectively.The parameters of average image intensity(AII),normalized AII(AII%,the AII ratio of IVS and LVPW ) and peak to peak intensity (PPI)in IVS,LVPW of 3 groups mentioned above were measured by ultrasonic integrated backscatter.Measurement data was descripted by x±S and compared with variance analysis.Enumereation data was descripted by rate and compared withχ2 test.The relation betweent the myocardium thickness and myocardium mass index and parameters of integrated back- scatter was analyzed with correlation analysis.Results:The thickness of IVS and LVPW and the LVMI of group LVH were all higher than those of group NLVH and the normal group(p<0.01).The AII and AII% of IVS and LVPW in group LVH were higher than those of group NLVH and the normal group(p<0.01). The PPI of IVS and LVPW in group LVH was lower than that of group NLVH and the normal group(p<0.01).There were no statistical difference between the measurements of group NLVH and the normal group.The thickness of IVS moderately correlated with the AII of IVS (r=0.65,P<0.01).The LVPWd moderately correlated with the AII of LVPWd (r=0.68 , P<0.01)。The LVMI moderately correlated with the AII of interventricu1ar septum(r=0.62,P<0.01). The LVMI moderately correlated with the AII of 1eft ventricularposterior wall(r=0.66,P<0.01).Conclusion:Our study showed that there was no statistic difference between the integrated backscatter parameters of NLVH group and the control group,but obvious statistic difference between the LVH group and the control group.It showed that there was myocardium specify change and myocardium damage in group LVH. Integrated backscatter can be used as referentia1 index to assess severity of myocardial damage in the patients with primary hypertension and offered high application value to the clinical.
