Objective1. To compare and to evaluate the efficiencies of four kinds of TiWI fast scanning sequences (TSE, IR-HASTE, true-FISP, and SE-EPI), and the efficiencies of three kinds of TiWI fast scanning sequences (FLASH, turbo-FLASH, chemical shift imaging) in determining hepatocarcinoma.2. To establish a new interventional radiological method-magnetic resonance imaging during splenoportography (MRSP), and to primarily assess its application in diagnosing of liver solid masses.Materials and Methods1. Thirty-four cases of hepatocarcinomas were undergone with the four T2WI sequences. Thirty-eight cases were undergone with the three T1WI sequences. All images were classified qualitatively and quantitatively. Signal intensities of liver lesions and surrounding hepatic parenchyma, background noise standard deviations were measured respectively. Then SNRs and CNRs of these sequences were calculated and compared. Four aspects of image qualities in lesion-liver contrast, construction of focal lesions, artifacts, and organ anatomy (mainly focused on the visualization of intraliver vessels) were used in analyzing the differences of the four sequences' image quality. Their determining rates of hepatocarcinoma were also evaluated.2. Sixteen patients with liver solid masses were performed with MRSP. SI of liver solid lesions, liver parenchyma and background noise standard deviations were measured in each phase of images. We then calculated and compared the images' SNRs and CNRs in each phase, and analyzed enhancement characteristics of liver parenchyma and focal lesion in different phases. The detection rate of MRSP was compared with US, enhanced CT and unenhanced MRI. We also analyzed eight perfusion abnormalities without liver solid masses in four patients during portographic phase, and accessed the effects of homogeneous enhancement of liver perfusion on the diagnosis.Results1. Compared with IR-HASTE and EPI, the SNR values of TSE and True-fisp are high with statistical significance(P<0.05, respectively). However, no significance was found in SNR value between TSE and True-fisp, EPI and IR-HASTE (P>0.05,-6-respectively).The CNRs of TSE and EPI were significantly higher than that of IR-HASTE andTrue-fisp(.P<0.05, respectively). No significant difference in SNR was detectedbetween TSE and EPI, IR-HASTE and True-fisp(P>0.05, respectively).In detecting focal lesion construction and artifacts, no significance was foundamong the four sequences(P>0.05, respectively). However, there are significancesbetween the four sequences on lesion-liver contrast and organ anatomy (P<0.05,respectively). TSE was the best one among the sequences being evaluated.No significance was found in the detection rates of TSE, IR-HASTE, true-FISP andSE-EPI whose detection ratios were 52(96.3%), 47(87.0%), 52(96.3%) and53(98.1%) among 54 lesions in 34 patients, respectively. (P>0.05).2. Compared with FLASH, IP-FLASH and OP-FLASH, Turbo-FLASH's SNR value was lower significantly (P<0.05), respectively). No significant difference was found in SNR between FLASH, IP-FLASH and OP-FLASH(P<0.05, respectively). The FLASH'S and IP-FLASH's CNRs value were significantly higher than Turbo-FLASH's and OP-FLASH's, respectively.The CNR values are significant between FLASH and Turbo-FLASH, IP-FLASH and OP-FLASH, IP-FLASH and Turbo-FLASH (P<0.05, respectively). The CNR values between FLASH and OP-FLASH, Turbo-FLASH and OP-FLASH, FLASH and IP-FLASH had no significance (P>0.05, respectively).No significance was found between FLASH, turbo-FLASH, IP-FLASH and OP-FLASH on lesion-liver contrast, artifact, lesion construction and organ anatomy(P<0.05, respectively). The detection ratios of the four methods were 55(93.2%), 54(91.5%), 53(89.8%) and 53(89.8%) in 59 lesions on 38 patients and no significance was found among them(P>0.05).3.
