| BackgroundSelective portal vein embolization (SPVE) as a kind of auxiliary treatment for liver malignant tumor has been widely used in clinical. Ultrasound-guided percutaneous selective Portal vein embolization has become one common operation technical of SPVE for its simple operation, no radiation, little pain, faster recovery. But its shortcomings lies in that, Due to tumor of oppression, tumor inner circle of disorder, severe liver cirrhosis, portal hypertension, congenital portal venous malformations factors, often leading to portal vein blood flow appears unusual, improper operation easy to the vagus embolization. The effect of color doppler ultrasound for guidance SPVE is limited. Some scholars has researched the blood flow of portal vein with the CO2 as ultrasound contrast agents. But the CO2 is a free gas, exist some shortcomings as big and difficult to pass imperceptible blood-vessel, poor stability. Another CO2 also failed to as a kind of ultrasound contrast agents on sales and limits its clinical application.In recent years, along with the rapid development of ultrasound contrast agents, plus ultrasonic imaging technology unceasing progress, make the contrast-enhanced ultrasonography(CEUS) has achieved great development, But the current clinical application mostly in ultrasonic diagnosis or treatment efficacy judgment. Study of CEUS by inject contrast in portal vein is rarely.In this study, we make ultrasono-portography using SonoVue through the portal vein embolization process and use it to guide SPVE, To explore the value of ultrasono-portography using SonoVue in SPVE.MethodsTwenty-eight patients with malignant tumor of liver were underwent percutaneous ultrasound-guide SPVE. And eleven of them were performed under the direction of colour doppler ultrasound(conventional group), the other seventeen were performed under the direction of ultrasono-portography using SonoVue(contrast group). CECT were performed after SPVE 2-4 weeks to realize the effect of embolization.ResultsThree patients were give up SPVE because ultrasono-portography show they didn't fit this therapy. The CECT showed that 2 patients of the conventional group had ectopic embolization, all cases of contrast group were no ectopic embolization.ConclusionApplication ultrasono-portography using SonoVue in SPVE, is effect for us to understand the blood flow and blood-supply area of the portal vein branch, the spatial relationship between the blood-supply area and tumor,the degree of embolization. ultrasono-portography using SonoVue provide important guidance for SPVE. |
