PURPOSEOptical Coherence Tomography (OCT), a kind of medical science reflection technology appeared recently , has been paid great attention because of its the non- contact ,non- invades , high resolution as well as wide application.The development of surgical technology and instrument equipment has led to a better surgical treatment of Rhegmatogenous Retinal Detachment (RRD) and a higher cure rate of retinal reduction.However, the regular checking methods such as the direct ophthalmoscopy, the slit lamp microscope, three mirror contact lens,binocular indirect ophthalmoscopy and fundus fluorescein angiography have their limitation. In order to assess the operation on RRD objectively and accurately , we study the RRD with the technology of OCT.METHODSTo observe the morphologic characters of macular in 62 cases 124 eyes (including 62 normal eyes) with RRD using OCT. Among them 41 cases 82 eyes (including 41 normal eyes) with the anatomic reduction and normal macular were scanned and measured by OCT.The thickness of the neurosensory retina and RPE and choriocapillaris thickness on given spots were measured and noted. The procedure is divided into three parts including the normal eyes,the unnormal eyes being in 6 or 12 weeks aftersurgery. Four scan lines traversing the fovea with the same length and the same angle gap were performed as the basic scan in every eye. additional scan were selected according to individuals including changing the length or angle of the scan lines. OCT is a new way for observation of RRD.RESULTSThe OCT imaging could be defined as three categories of macular changes in RRD before surgery. (1) The macula was attached in 29 eyes; (2) partial macular detachment in 5 eyes including cystoid edema of the neurosensory retina in 2 eyes and with vitreous adherence in 3eyes; and (3) total macular detachment in 28 eyes including network edema of the neurosensory retina in 18 eyes, retinoschisis in 3 eyes, with macular hole in 5 eys, and epiretinal membrane in 2 eyes.After surgery normal retina was found in 41 patients,anatomical structures reduction in 7 patients include secondary premacular membrane in 3 patients and honeycomb form edema in 5 patients, macular scar in 2 patients, markedly thinned neurosensory retina in 3 patients, dissecting the structure reduces unentirely in 5 patients with extraretinal fluid, detachment of neurosensory in 3 eyes with retinal fluid.The data showed that the thickness of neurosensory retina of 6W and 12W after the RRD operation in 41 cases were coincidernt with the anatomic distribution of the nerve fibre of macula.The data collected at the center, 50 u m and 150祄 which are usually regarded as data acquisition points in the macula lutea center fovea scopes (0.3-0.35mm) were very close in the different directions or different distances, and the variation is smaller; But there is a biggest variation among the individual at the 500 n m point ( standard deviation is the biggest ) ; Although the mean and the standard deviation in 1000 ?m and 1500 u m position are comparatively close to each other , the mean changed obviously in the different directions of same distance , and the mean of temporal side is minimal .The comparisons each other in three tables showed that the data from patients of 6W after operation were bigger than those of 12W after operation and healthy eyes while the data were close to those of 12W after operation and healthy eyes. The statistics indicated that the thickness of the neurosensory retina from same standard and same direction exists the significante difference (P value < 0.05). while there is no significance difference ( P value > 0.05) between the healthy eyes and 12W after operation. The thickness of RPE and choriocapillaris in corresponding9position of macula lutea as well as the statistics showed that the thickness average value on different position is comparatively close, and the standard deviation is obvious.
