Coronary heart disease (CHD) is one of the common cardiovascular diseases seriously threatening the health and the life of people . The morbidity is gradually increasing yearly. Acute coronary syndrome(ACS) is the severe clinical manifestation of coronary heart disease, which include ST-elevation acute myocardial infarction (STAMI), non ST-elevation acute myocardial infarction (NSTAMI) and unstable angina pectoris (UA). The study on the mechanisms of ACS is the focus all over the world by now, but its clear mechanism remains unknown. Recent years more and more attention to the relation of inflammation and ACS has been paid. Acute coronary events was associated with the rupture of plaque and the thrombosis, inflammation may be the pivotal causal factor. So in our study the levels of blood C-reactive protein (CRP), Fibrinogen(FBG), troponin I (cTnl), Creatine phosphokinase(CPK), Creatine phosphokinase isoenzyme(CPK-MB), Lactate dehydrogenase (LDH), a-Hydroxybutyric acide hydrogenase(aHAD),Glutamic-oxal(o)acetic transaminase(GOT) were assessed in patients with acute coronary syndrome(ACS) compared with the control group which involved patients with stable angina pectoris (SA), old myocardial infarction(OMI). The aim of this study is to further clarify the role of CRP and FBG in the the pathogenesis of ACS.Rate nephelometry with the Beckman systems was used to measure the plasma levels of CRP, FBG, cTnI, CPK, CPK-MB, LDH, a-HAD, GOT in 75 patients with ACS(25 of STAMI, 10 of NSTAMI, 40 of UA) and none- ACS(74 of SA , 52 of OMI).The peak levels of CRP, FBG, cTnl, CPK, CPK-MB, LDH, a-HAD, GOT in ACS group are significantly higher than those of control group (P<0.01). The peak levels of CRP, FBG, cTnI, CPK, CPK-MB, LDH, a-HAD, GOT in STAMI and NSTAMI groups are also obviously higher than those of control group (P<0.01) . Although the levels of CRP, FBG in UA group are significantly higher than those of control group (P<0.01), the levels of cTnI, CPK, CPK-MB, LDH, a-HAD and GOT in UA group didn't significantly increase compared with none- ACS group (P>0.05) . The level of CRP in ACS group is positively correlated with the peak level of cTnI, CPK, CPK-MB, LDH, a-HAD and GOT (P<0.01) , the level of FBG in ACS group is correlated with the peak level of a-HAD.Conclusions1. The plasma levels of CRP and FBG in ACS group significantly elevated comparedwith the non- ACS2. The plasma level of CRP in ACS group were associated with the severity of the disease(myocardial damage) , indicating that CRP play an important role in the pathogenesis of ACS.
