PURPOSE: To investigate the characteristics of cognitive function disorders of multiple sclerosis(MS),as well as the correlations between cognitive dysfunction and the changes of event-related potentials, neuroimaging.The related factors of congnitive dysfunction in MS were explored.METHODS: 80 patients with clinically definite MS were studied with the event-related potentials(ERPs), among them,70 patients were test with the Wechsler Intelligence Scale(WIS). Analysis the brain MRI of 50 patients, DTI of 5 patients and PET of 2 patients.The incidences of cognitive dysfunction were studied.The relationship between cognitive dysfunction and the changes of event-related potentials. MRI and PET was analysed.RESULTS:l.The verbal intelligence quotient(VIQ),performance intelligence quotient(PIQ),full intelligence quotient(FIQ)of patients with the IQ less than 90 scores were 37.2%, 45.7%, 40% respectively,which were higher than those of the normal group (P<0.01).The scores of IQ in MS of cerebral type or cerebro-spine type and of chronic-progressive type were less than those of the spine type and replapsing-remitting type(P<0.01). Multiple-regression analysis demonstrated that the lesions of brain was the by far strongest influence on IQ.2.The latency of N2,P300 of event-related potentials(ERPs) was significantly longer in MS than that of in the normal controlgroup(P<0.05). The amplitude of P300 were also lower and had poor morphologies in MS.The positive correlation between P3 latency and the disease duration was significant,as well as the positive correlation between P3 amplitude and IQ.The negative correlation between P3 latency and VIQ,FIQwas also significant.3.The MRI of patients with MS show the total lesional score was significantly correlated with the duration of the disease. There was a negative correlation between the numbers of lesions in corpus callosum and FIQ ( P<0.05 ) .The P3 amplitude and PIQ were significantly different between the patients with corpus callosum lesions and the patients without corpus callosum lesions. The score of PIQ of the patients with frontal lesions was lower than that of the patients without frontal lesions.The ventricular exponent VBRand the third ventriclar exponent of patients with the score of FIQ<90 were more severe than patients with the score of FIQ 90. Spearman analys between IQ and MRI parameters show BCR and the third ventriclar exponent were the best parameters,then CCR,and the numbers of lesions was the last. The agents of WIS such as completion of drawing, block pattern,figures arrangement were related with five of the MRI parameters.There were significant correlations between P3 latency and the ventricular exponent,VBR and the third ventriclar exponent,so as well P3 amplitude and the BCR.4.The DTI of five patients show the ADCs measured in the lesions were significantly higher and the FAs were significantly lower than the white matter regions in control subjects.The ADCs of periplaque white matter(PWM),NAWM and the normal-appearing gray matter((NAGM) were also higher,and the FAs of PWM,NAWM,NAGM were lower than the white matter regions in control subjects.This indicated that there were pathologic changes in those tissues.5.The PET of two patients show there were many hypo-metablism areas in cortex.In addition,some cortical hypo-metablism areas which were found by PET were not found by MRI.The hypometablism areas in cortex were strong correlated with the cognitive dysfunction of MS.CONCLUSIONS:1.Cognitive impairment was a common occurrence in MS, especially to those patients who belong to cerebral type or cerebro-spine type or chronic-progressive type. The locus of lesions was an importance factor to the score of IQ.The lesions of brain was the by far strongest influence on IQ.2.The latency of N2,P300 of ERPs was significantly longer in MS than that of in the normal control group.The amplitude of P300 were also lower and had poor morphologies in MS.
