Objective To investigate the effection due to warm ischemic reperfusion injuryin rat small bowel transplantation, study the restitution and regeneration mechanisms, explore the relatively effecting factors of warm ischemia reperfusion injury during graft harvest and the evaluation significance.Methods Heterotopic small bowel transplantation models were established with inbred rats F344 by microsurgical technique. The rats were divided into 5 groups(N=6/group) including warm ischemia time 0, 15,. 30, 60, 120min. The peripheral blood were taken to assay Nitric Oxide (NO), endothelin-1 (ET-1), superoxide dismutase(SOD), malondialdehyde(MDA) on series pionts of respectively postoperative day(POD) 1, 3, and 7. Simultaneously, samll bowel biopsis were gotten for routine histopathological examination and RT-PCR assay, of intercellular adhesion molecule-1 (1CAM-1). The survival rate of recipent rats and grafts were observeted.Results 1. The score of histopathology of grafted small intestinal mucosa was elevatvated with increasing of warm ischemia time on POD1. 2. It is demastrated that small intenstinal mucosa of each warm ischemia groups were incompeletely recovered, the differences of mucosal injury among those groups were disappear, and the almost coincidencing scores of histopathology were exhibition on POD7. 3.There were increasing of the level of NO, ET-1, MDA inrecipients peripheral blood, meanwhile, decreasing of the concertration of SOD and the ratio of NO/ET-1 on POD1. These alterations were not significant on POD3 and 7. 4. The compacting correlation between the ratio of NO/ET-1 and the score of mucosal lesions was showed on POD1.Conclusion l.The injury of reperfusive small intestinal mucosa following rats small bowel transplantation was gradually serious with enlonged warm ischemia time during 15 min to 120 min. There is no significent effect on repairment and regeneration of grafted small intentinal mucosa by increasing warm ischemia time tent. 2.In warm ischemia groups, the obvious morphological repairment of mucosa in transplantated bowel could see on POD3 evidently, but no complete recovery on POD7. 3.It plays the key role in early changing of warm ischemia reperfusion injury that are NO, ET-1, MOD, SOD, ICAM-1. 4.The ratio of NO/ET-1 could be one of valued index to predicate the critic of ischemia reperfusion injury.
