PrefaceIn the clinical practice and animal experiments of esophageal surgery , ER is usually used to deal with esophageal disease. Anastomosis cant be applied directly between the cut ends of the esophagus because its segmentary blood supply so that we have to select suitable substitute to rebuild the continuity of the alimentary canal. Many substitutes are used to rebuild esophagus, such as; 1. Autogenous tissue ( 1) autogenous tube tissue: stomach, jejunum, colon and so on . (2) autogenous non - tube tissue : kinds of cutaneous flaps and muscular flaps. 2. Artificial esophagus. There have been many ways to rebuild esophagus by now, but they are still to be developed and improved.Using autogenous tissue to rebuild esophagus is one of the recent research aims . This test is to rebuild esophagus by autogenous pulmonary tissue and to observe the postoperative symptoms, pathological changes , so as to find the feasibility of a new esophagus substitute which is convenient ,safe and less likely to be damaged and also accords with physiology . We also provide the theoretical evidence of this new method by pathological research.Methods and Materials1. Materials1. 1 animals for experiments : 24 healthy adult mongrel dogs provided by our animal experiment center , male or female , 10kg to 20kg by weight1. 2 Usual drugs and reagents provided by our animal experiment center1. 3 Appliances : operation bed , anesthesia appliance , trache-al intubator, self - expanded alloy stand , microscope, transmission e-lectron microscope.2. Methods2. 1 Operation methods : Neither food or water is supplied on the morning for the operation . The dog is anesthetized by sodium thio-pental (30mg/kg) through injecting peritoneal cavity. Trachea! intubator is given and its breath is maintained by anesthesia appliance. Gastrostomy is applied at first and we burrow into the right thorax between the fifth and sixth ribs . Middle bronchus is ligated but isn' t dissociated so that the vena and artery of pulmonary can be protected at the same time , thus the PF with vessel is obtained . A 4 - centimeter - long and 1/2 to 2/3 perimeter defect is made in esophageal wall , and then rebuild esophagus only with PF (team A) or repair it using PF with the self - expanded alloy stent inside (team B). Besides the method of inserting the stent , the others are the same . At last , we bundle pleura , wash thorax , set up thorax drainage tube , suture lays of chest tissue . Postoperation tendance : remain thorax drainage tube for 2 - 3 days , observe the amount of drainage , and after it reduces , pull out the tube , eliminate inflammation for 7 days by injec-ting penicillin 400 7? u qd. iv. drop , feed the dog on thorough fluid food till the seventh day , then feed the dog on semifluid food till the fourteenth day and then on common food . We also observe the changes , usual condition and weight .2. 2 Observation method : gastrofiberscope and chest x - ray are applied at different interval after the operation . After being executed or dying naturally , the esophageal substitute in the dog is taken out . Observe pathology morphologic change and make pathologic study.Pathological section making ; The specimen is fixed by 10% Formalin, gradient dehydrated by 75 - 100% Alcohol, treated by xylene , embedded by paraffin and then section is made . At last the section is stained by eosin and haematoxylin.Section for electron microscope making : The specimen is fore-fixed by glutaraldehyde , poached by buffer solution , afterfixed by os-mic acid , gradient dehydrated by Alcohol and acetone , treated by embedding agent and polymerizated in the incubator . At last we tidy up the pieces and ultramislice is completed.Result2 weeks after operation; we find that the PF and the ends of e-sophagus have healed well by fibroid tissue. The growth of esophageal epithelium are observed from periphery area towards central area. 8 -10 weeks after operation : The surface of defect in team A is covered by new - born epithelium tho...
