Inferior Pedicle Reduction Mammaplasty Without Vertical Scar

Mammary hypertrophy and ptosis is one of common diseases in female. It make women have bad outlook and feel indisposed. The aim of reduction mammaplasty is to resect the unnecessary skin and glandular tissue of breasts so that the size is reduced, the outlook is correct and nipple-areola complex is rebuilt. In 1986 Han Jianqun reported the inferior pedicle reduction mammaplasty without vertical scar. 15 patients have been operated with this technique in our department since 1994. Material and methods: we observed the changing of their breast contour , lysis of their symptom and contrasted their bloodloss and operation time with other 15 patients undergone the inferior pedicle reduction mammaplasty with vertical scar. Design of operation: let patients stand. Decide new nipple location accurately by several methods. The width of inferior pedicle is no less than 8cm . Draw an arced line which is 5cm-6cm distance from the inferior border of new areola .In the base of inferior pedicle , design an arced area. Anesthetic method: continuous epidural anesthesia. Technique of operation: Resect epithelium of the inferior pedicle except the area of nipple-areola and the arced area. Resect useless skin and glandular tissue included that in the new areola . Lift the inferior pedicle ; bring the nipple-areola complex out from the area of new areola . suture the incision around the areola and in the inframammary . Results: Symptoms of 15 patients disappeared after operations .New breasts were hemisphere . The nipple-areola complexes were sensitive . Scar was not obvious , especially in the arced area of inferior pedicle. Statistic results about tow operations is significant. Conclusions: Keys of reduction mammaplasty are to reset nipple-areola complexes accurately ; maintain enough blood supply of nipple- areola complex and rebuild aesthetic contour of breast . With the width of the inferior pedicleno less than 8cm and thickness of the glandular tissue under the inferior pedicle no less than 1. 5cm, the nipple-areola complexes will have plenty blood supply and nerve control. Design of the inferior pedicle reduction mammaplasty without vertical scar is standard and its operation is convenient . This operation eliminate the vertical scar , release the tension of inframammary line incision .The breast shape after operation is full and round ; the scar in incision is not obvious ; the operating time is shorten; the blood loss is reduced.
