Objective: To discuss different methods of operation for development dysplasia of the hip.Methods: 98 patients (132 hips ) with the age ranging from 2to 15 were operated from 1987 to 1999. 62 patients with 76 hips ( Salter innominate osteotomy 28 cases,Chiari osteotomy 22 cases,Pemberton acetabuloplasty 20 cases,hip-shelf procedure 6 cases ) were followed up for 3 to 15 years,an average of 11.2 years. We use the clinical percentage criteria raised by the Suzhou No.l Hospital affiliated to Suzhou University and radiological guidelines of the Severin classification system to assess the different operations.Results:61 cases have the score over 80 and the satisfaction rate is 80.3%. With the statistical examination to compare the score before and after operation,all the operations have sound effects on the development dysplasia of the hip and have no statistical meaning on the different methods. The curative effects maintain close ties withthe operation. After the operation 63% of the patients rated as Severin Class 1,31% as Class II,4% as Class III and 2% as Class IV.Conclusion:1. The children are treated mainly with the operation. The curativeeffects maintain close ties with the operation. The earlier diagnose and treatment,the better results.2. The preoperative traction has little value on the treatment. According to our experiences,it need not to be used.3. All of the methods of the surgery have sound effects on the DDH. We could not say which one is better. AH have their indications. It is approved that the Salter innominate osteotomy has a positive results of the children under 6. But it is not recommended to treatthe children over 7. Chiari osteotomy adapted to almost all of the older children.4. The femoral shorting,derotation osteotomy is used to correct the anteverted angle and help the reduction of the congenital dislocation of the hip.5. All of the operations methods have their classical sequences. The operator could do some improvement according to his experiences to make the procedure easier and more effective.
