Total Hip Arthroplasty Combined With Short Femoral Osteotomy For Crowe Type ? Hip Dysplasia

Objective: To discuss the method and effect analysis of total hip arthroplasty(THA)and femoral intertrochanteric shortening osteotomy for the treatment of Crowe type ? hip dysplasia(DDH).Methods: From October 2015 to January 2018,14 patients(16 hips)with Crowe type ?hip dysplasia(DDH)were admitted.There were 2 males and 12 females,including 12 patients with unilateral hip dysplasia,Two patients with hip dysplasia,mean age(52 ±5.5)years.The use of acetabular acetabular reconstruction in the level of real acetabulum,transverse femoral trochanter transverse row shortening osteotomy,femoral bioprosthesis total hip arthroplasty.Among them,14 patients were treated with ceramic-topolyethylene acetabular interface prosthesis.Postoperative patients with bilateral hip joint DR positive position,double lower extremity standing full length and other imaging data combined with Harris hip evaluation system(harris hip score,HHS)in patients with hip function score.Results: Two cases were followed up for 12 cases(14 hips).The HHS score of the last follow-up postoperatively was significantly higher than that before operation(P <0.05),and the hip mobility increased after operation.The hip joint Pain improved significantly,walking close to the normal gait.There was no incision infection.During the follow-up period,the acetabular prosthesis had good osseous coverage,no loosening and sinking of the prosthesis,and the osteotomy ends healed in 3 ~ 6 months after operation.Couclusion: THA combined with transverse truncated femoral trochanter is especially suitable for Crowe type ? DDH patients with large difference in the length between the two lower extremities.It can effectively reduce the difficulty in the reduction of the hip joint and the length of the sciatic nerve Risk of vascular injury,it is worth clinical promotion.
