Objective: Hypertension, one of the common diseases, is a risk factor of stroke and coronary heart disease. The key of controlling blood pressure is adherent to hypertensive treatment. While reported drug compliance rate to hypertensive is only 20%~50%. To study the compliance status and influence factors in the different group of patients with hypertension and give some suggestions for improvement of the compliance, we conduct this investigation.Methods: A questionnaire survey and blood pressure measure were conducted in the three group of hypertensive patients from a rural village, an urban community and a research institute . The data investigated included: social demography data, hypertension and therapy conditions, the knowledge about hypertension and attitude to hypertension, r-test, chi-square test and multivariate Logistic regression analysis were used for the analyses of the general condition and influence factors of compliance.Results:1. The rate of high blood control is 11.9% and drug compliance rate is 36.1% in 194 eligible questionnaires from a village ( mean age:61.59; median duration of disease course: 4.0 years ) . The multiple Logistic regression analysis shows that knowledge about hypertension and course of disease are significant factors of drug compliance.2. The rate of high blood control is 27.7% and drug compliance rate is 51.1% in 184 eligible questionnaires from a city community ( mean age:70.76; median of disease course: 11.0 years ) . The multiple Logistic regression analysis shows that pattern of medical expanse and frequency of blood pressure measure are significant factors of drug compliance.3. The rate of high blood control is 55.5% and drug compliance rate is 55.5% in 245 eligible questionnaires from a research institute ( mean age: 56.49; median of disease course: 10.0 years ) . The multiple Logistic regression analysis shows that age , frequency of blood pressure measure , drug effect , influence to the daily life and understand to hypertensive harm are significant factors of drug compliance.Conclusion: The influence factors are varied in the three groups. The pertinent suggestions should be put forward to improve the compliance aimed to different people. In the village patients the key to improve the drug compliance is to hold health education and increase their knowledge about hypertension. The blood pressure should be measured regularly in the elderly in the city community.For the patients in the research institute, who have good health insurance and with good salary and high level education, compliance for the younger patients should be concerned.
