The Studies on Taking L Value as a Diagnostic Criterion for SAS and on Orthognathic Surgical Treatment of the Patients With SAS Due to TMJ Ankylosis and to Evaluate the Curative Effects by Means of L Value In the patients with sleep apnea syndrcxne (SAS) due to ankylosis of temporomandibular joint, their developmental deformities often cause mandibular retrognathia which greatly affects the original physiological function of the stomatognathic system and the respiratory tract. Orthognathic surgery technique can make the mandibular body and the muscles attached to the bone move forward. The respiratory tract expands accordingly with the movement of the jaw bone. In this way, SAS can be treated effectively. As a classical analytical method of craniofacial figure, measurement of the cephalometric roentgenogram has its limitations. In recent years, with the advances in science and technology, computers are widely used in medical works Spiral computerized tomography, which can make three-dimensional image available, is having vide clinical application. It is a promising analytical meth4 for it can analyze the craniofacial figure and the upper respiratory tract in a reliable and precise way.The pmposes of this study are as follo: 1. To measure the vokune of pharyngeal cavity and the area of transverse section of the 6th cervical vertebrae of both healthy people and patients with SAS, using spiral computerized tomography. The ratio of the volume of pharyngeal cavity to the area of the 6th cervical vertebrae is used as index to see Wit can be used to diagnose SAS and to evaluate the curative effects alter SAS operations. 2. Patients with SAS due to ankylosis of tempommandibular joint undergo arthmplasty at the affected side, type "L"semi-layer osteotomy at the posterior border of the ramus of mandible and the mandibular body, as well as sagittal osteostomy at the normal side. The mandible is pulled forward and the rigid internal fixation is strengthened. The post-operative effects are evaluated.Part I. A Study on Taking L Value as a Diagnostic Criterion for SAS Objecttive: To evaluate L value (the volume of pharyngeal cavity/the area of transverse section of the 6th cervical vertebrae) and see if it can be used as a diagnostic criterion for SAS. Methods: The volume of pharyngeal cavity and the area of transverse section of the 6th cervical vertebrae of both healthy people and patients with SAS were measured using spiral computerized tomography. The data were analyzed statistically. Results: L values of healthy people and that of patients with SAS are significantly different. The positive correction was found between L value and Al-LI. Conclusion: L value can be used as a good diagnostic criterion for SAS instead of AHI. It is characterized by quickness, simplicity, and accuracy.
