Objective : Objective to study the MRI changes of temporomandibular joint (TMJ) in mild and moderate OSAHS patients before and after 18 months, and to investigate the effect on the temporomandibular joint reconstruction.Methods: twenty patients with mild to moderate OSAHS were enrolled, including 17 males and 3 females, aged 25-59 years. All the patients were treated by metal rods of mandibular advancement snore stopping devices, respectively, MRI scan of bilateral temporomandibular joint was performed before and 18 months after treatment. The oblique sagittal images of the temporomandibular joint were selected and the position of the articular disc was evaluated by the hour method. by Kurita analysis before and after the treatment,the relationship between the position of condyle,articular disc and articular fossa; Oblique coronal observation of articular disc without lateral displacement. We used the SPSS21.0 software for statistical analysis pairedt-test of the recorded data.Results:1. The twenty cases of OSAHS patients with MRI examination can clearly display the anatomical structure and relative position of temporomandibular joint and soft tissue.2. The twenty cases of the patients with the OSAHS was ( 8.50± 2.07°), on the left side of the anterior joint disc,and the right position was( 8.53 ± 1.15 °). After MASSD treatment for 18 months,the left position of the articular disc was( 8.92± 1.50°), and the right position was (8.95 ± 1.39° ). compared before and after the treatment, there was no significant difference between the disc position (P>0.05).3. The condyle and glenoid fossa position (TC/TP), articular disc and glenoid fossa position (TD/TP), the relative position of the articular disc and condyle (TD/TC) before and after treatment showed no significant statistical differences (P>0.05).4. Oblique coronal observation of the articular disc margin beyond condylar phenomenon.The inner edge of the disc condyle inner space before and after the treatment (ac) and outer disc condyle spacing (bd) without significant difference (P>0.05).Conclusions:1. MRI examination can clearly showed the anatomic structure and the mutual position of the soft and hard tissues of temporomandibular joint, and have certain clinical significance for the diagnosis of temporomandibular joint disease.2. Patients with mild and moderate OSAHS wore a mandibular advancement snore stopping device. There was no significant change in temporomandibular joint before and after treatment for18 months. It indicated that the treatment had no effect on the reconstruction of temporomandibular joint.
