Objective To investigate the relationship among ACE gene l/D polymorphism, hypertension and cerebrovascular disease, and to find the cause and pathogenesis of hypertension and cerebrovascular disease. Methods ACE gene I/D polymorphism was analyzed by PCR- AFLP method in cases of 40 essential hypertension, 45 lacunar stroke, 54 cerebral infarction, 33 cerebral hemorrhage and 66 healthy controls. Plasma Ang II levels were determined by radioimmunoassay in 54 cases of acute cerebral infarction and six of them were examined again after about twenty days. Results The genotypes of ACE gene I/D polymorphism were confirmed: DD, DI and II. The frequencies of DD genotype and D allele were not significantly higher in the cases of hypertension, lacunar stroke, and cerebral hemorrahage than in the controls. The frequencies of DD genotype and D allele were as follows: 22.2% and 45.4% in cerebral infarction patients, 13.6% and 3 2.6% in controls, respectively. Compared with the control subjects the frequency of DD genotype in cerebral infarction was not of significant difference, but the D allele frequency was significantly different (P<0.05). The plasma Ang II levels in cerebral infarction patients with DD genotype were significantly higher than those with II genotype(P<0.05).The plasma Ang II in the patients who were examined twice were 31.33 ?5.49 and 25.20 ?1.44. The difference between them was significant(P<0.05). Conclusions These suggested that D allele was not the risk factor for hypertension, but it was a risk factor for cerebral infarction. The DD genotype increased risk of cerebral infarction and investigation of ACE genotype among these persons might be of help for prevention of cerebrovascular disease. The cerebral infarction patients with DD II genotype had higher plasma Ang II levels The plasma Ang II levels were higher during the acute time in the cerebral infarction patients.
