Objective:To improve the level of rational clinical medication in our hospital, and to provide references for further clinical pharmaceutical care in pediatrics.Method:In 2014, the author participated in the clinical pharmaceutical practice through the following ways to improve the drug therapy of antibiotics and glucocorticoids:① Prescription comment:Clinical pharmacists brought a retrospective review into force from a certain number of pediatric prescriptions monthly by HIS including outpatient, emergency, hospital and vancomycin prescriptions. ② Pharmaceutical care:Clinical pharmacists carried out the pharmaceutical care which focused on the seriously-ill children to close observation of the changes and response.Then at any time clinical pharmacists adjusted the medication to ensure the safety and effectiveness of the children. ③ ADR monitoring:Clinical pharmacists implemented the ADR monitoring system, and took measures when there’s something urgent in medicine. Besides that, we strengthened the analysis of factors to minimize the occurrence of drug-induced ADR. ④ Pharmaceutical education:Clinical pharmacists provided pharmaceutical education for pediatricians, nurses and patients. Furthermore we issued to consultants. Afterwards administration of pediatric clinical indicators was analyzed by Excel and SPSS, then we calculated the passing rate of prescription and the incidence rate of ADR, the satisfaction of paediatric s’family was assessed by questionnaire, etc.Result:In 2014, many pediatrics clinical indicators significantly improved. Qualified rate of hospital prescription increased from 81.6% to 90.2%, outpatient and emergency prescription increased from 91.7% to 95.7%, qualified rate of glucocorticoid prescription increased from 68.4% to 88.5%. The satisfaction of paediatric s’family improved significantly, moreover the effect of pharmacy service in pediatrics is remarkable.Conclusion:Through the clinical pharmacy services in depth, antibiotics and glucocorticoids in pediatrics of clinical rational use level were improved, which realized the pediatric clinical pharmacists’ value in pharmacy service. Through active participation in the practice of clinical pharmacy in pediatrics of our hospital, the professional skill was improved, and it is important for me to realize the clinical pharmacists responsibility and mission. In this environment under the influence of laying stress on clinical pharmacy, we should improve the professional quality endlessly as far as clinical pharmacists become an indispensable force in the medical team by degrees.
