A Systematic Review Of The Efficacy Of Oral Chinese Herbal Decoction In The Treatment Of Bronchiectasis And Its Medication

Objective1To systematically evaluate the efficacy of decoction of chinesemedicine in the treatment of bronchiectasis.2To explore the drug law oftraditional Chinese medicine in the treatment of bronchiectasis.Data sources1system evaluation:2reviewers retrieved China CNKIdatabase,CBM,Chinese scientific journal database (VIP), Wanfangdatabase,PubMed. Each database date to2014January20th was the date ofretrieval. The randomized clinical trials with decoction of chinese medicinefor treating bronchiectasis as experimental group, only western medicine ascontrol group was included in.2people independently extracted data.If therewere differences, two people solved the problem through consultation.2clusteranalysis: First prescriptions in the treatment of bronchiectasis were collectedfrom CNKI database so far.The original database was established using Excel.Drugfrequency was analyzed and a database was established using the drug use ofhigh frequency.Method1system evaluation: Included literatures were analyzed using RevMan5.2software.Heterogeneity across studies wae determined.Effect model was selectedaccording to the heterogeneity. The end effect index was showed by odds ratio(OR)or mean difference(MD) within95%confidence interval(CI). Publication bias wastested using funnel plot.2cluster analysis:Drug times were analyzed usingExcel. The drugs which the times≥10were chosen to do frequency analysis,cluster analysis,and sort the drugs by effect. Database of The high frequencyof drugs was established using Excel. Each recipe includes variables (drugs)were turned into two value variable. The drug was set to1, no was set to0.Cluster analysis was done using spss18.Results1system evaluation: Total of16articles were included,and1433patients were involved. curative effect of this16studies were evaluated.OR=4.28，95%CI=[3.03，6.05]，Z=8.22、P<0.00001;7studies about curative effect of syndrome of traditional Chinese including cough, expectoration and so on wereevaluated.OR=3.93，95%CI=[2.29，6.75]，Z=4.97、P<0.00001; Medicine As a standardof curative effect in accordance with the "guiding principles of clinicalresearch on new drugs of traditional Chinese medicine in the treatment ofhemoptysis", curative effect of4studies were evaluated,OR=5.38，95%CI=[2.37，12.18]，Z=4.03、P<0.00001;2studies were analyzed about hemostatic time.MD=-2.56，95%CI[-3.59，-1.54]，Z=4.90、P<0.00001;2articles reported interleukin-8(IL-8) after treatment,MD=-9.82，95%CI[-11.71，-7.92]，Z=10.14、P<0.00001;2cluster analysis: Drug treatment of bronchiectasis included phlegm andantiasthmatic, tonic medicine, antipyretic, hemostatic, diuresis medicine, Qimedicine, promoting blood circulation and removing blood stasis drugs, relievingdrugs most commonly used. It can be divided into11categories by clusteranalysis.Limit1system evaluation: The methodological quality of16studies was not high.the studies using other languages, manual retrieval, gray literature, forexample unpublished data, special reports, government reports and othertraditional or non-traditional literature sources of evidence were not included.Sample size of all studies was less, Influence of the accidental factors weredifficult to exclude. This study described the effective rate all aboutimprovement of symptoms, not included clinical index, it was lack of convincing.This study was lack of long term follow-up and safety evaluation.2clusteranalysis: Increasing the number of cases can improve persuasive. Clusteranalysis has its limitations,so that objective,precise and uncontested resultscan not be got.Conclusion Decoction of Chinese medicine can effectively improved the symptomof bronchiectasis. Drugs is mainly heat, phlegm, deficiency, hemostatic medicine.It Can be divided into11categories, including Shugan Jieyu, resolving phlegmhemostasis; cooling blood to stop bleeding; Jianpi Huatan; clearing heat andresolving phlegm, eliminating carbuncle abscess; nourishing lung and stomach;clearing heat and eliminating swelling; liver and lung, cooling blood and phlegm.
