ObjectiveThe key of acupuncture effect lies in the effective stimulating amount and duration of stimulation. Acupuncture is widely used in the treatment of lumbar disc herniation (LDH),but the curative effect is not necessarily positively correlated with the time of needle retaining. With the purpose of comparing the probable relationship between needle retaining and therapeutic effectiveness, the study attempts to compare the different therapeutic effects of needle retaining time during the treatment of LDH,and analyze the proposed scheme of acupuncture treatment time combined with the cost efficiency in health economics. Therefore, it could offer an effective and specification reference for the treatment of LDH in clinic.MethodsThe study adopts randomized controlled experiment scheme. The 54 cases obversed were randomly divided into three groups according to the inclusion order. The needle retaining time is respectively 15minutes,30minutes and 45 minutes with the same acupoints generally used in clinic and acupuncture manipulation. The treatment was given twice a week and ten times were taken as a period of treatment.Before and after therapy the McGill Pain Questionnaire-short form,the Modified-Modified Schober (MMS), and the Chinese version of the Roland-morris Disability Questionnaire(CRMDQ)were used respectively as observation index to quantitative evaluate the degree of pain, the joint activity and the degree of dysfunction.,as well as the overall effect.Processing the data by statistics methods, and analyzing it for the different effects of different needle retaining time.Results1.Comparison of the pain scores in the patients of three groups:before and after treatment in every group,the difference in Pain rating index(PRI) scores and Visual Analogue Scale(VAS) scores were significant (P<0.01);2.Comparison of the joint activity in the patients of three groups:before and after treatment in every group,through the Modified-Modified Schober(MMS),the difference in joint activity were significant (P<0.05)3.Comparison of the degree of dysfunction:before and after treatment in every group.through the Chinese version of the Roland-morris Disability Questionnaire (CRMDQ),the difference in the degree of impairment were significant (P<0.01);4.Comparison of the pain scores,the joint activity and the degree of dysfunction after treatment:there was no significant difference among three groups (P>0.05);5.The total efficiency of three groups were respectively 68.75%(Group 15min),88.89% (Group 30min),94.44%(Group 45min), there was no significant difference between the effect of the three groups (P>0.05).Conclusion1. After one period of treatment,the pain degree and adverse emotional problems caused by pain was significantly reduced, the joint activity improved and the degree of dysfunction decreased in every group, suggesting that simple acupuncture treatment is an effective therapy for LDH which could alleviate pain and dysfunction sydrome.2.There was no significant difference in these index and the total effect among three groups, that is to say that,it has not been proved that needle retaining has an effect on the therapeutic effectiveness;3. Combining the relationship of the cost and efficiency of health economics as well as current health conditions and demandse in China, we could induce that if the conclusion above is correct, needle retaining for 15min can not only ensure the treatment effect but allowing more patients to receive treatment in a certain period of time, and reduce the extra cost along with acupuncture treatment.lt is worthy of clinical reference.4.Although after treatment there’s no significant difference in the observation index and the total effect of the three groups, as for the current trend, the total effective rate of groups with 30min and 45min needle retaining is higher than the group with 15min.With a small sample size it just provides a stage of cognition.We could suppose that if we increase the sample size, it would come to a different conclusion.the results.
