ObjectiveWe study the pathological changes of chronic atrophic gastritis and gastric mucosa pathological degree, and the relationship between both with TCM syndrome type, laying a foundation for clinical evidence-based medicine。MethodsBy analyzing the data of the whole 625 patients, the data base was established. Using the descriptive statistics, chi-square test and KruskalWallis test to analyze the relationship among age, gender, risk factors, medical history, family medical history and CAG. At the same time, summarize the distribution of different TCM syndromes of relevant pathological changes. For 511 cases with pathology sites to establish a database, we use thestatistical method to analyze the relationship between pathogenesis of location and pathological degree of chronic atrophic gastritis.Results1. The whole 625 cases conclude 253 men and 372 women, the ration is 1:1.5; the the oldest is 70-year-old, while youngest is 24; patients over 50 years old account for 64.80%.47.20% of patients come from Beijing among the 25 provinces and the Han nationality account for 97.28%. The top five risk lifestyle factors are emotional depression(33.44%), drinking(22.40%), smoking(20.96%), mental stress(20.00%) and catching cold(17.44%); the top 5 risk dietary factors are eating disorders(21.92%), sweet food(20.96%), peppery food(19.04%), acid food(11.04%), salt food(10.88%). For BMI, normal account for 57.92%, overweight account for 23.04%, obesity account for 6.24%, and the low account for 12.80%. The shortest course of CAG is less than 2 months, the longest is 28 years, and that less than 5 years is the most, accounting for 91.20%.Patients who have family medical history of CAG or gastric cancer take a percentage of 20.00. Patients of duodentits or duodenal ulcer account for 13.12%. Patients with H.pylori infection account for 56.48%.2. The frequency of different TCM syndromes are found to be:spleen-stomach deficiency syndrome(33.36%)> stomach meridian of blood stasis syndrome(14.88%) > stagnated heat of liver and stomach syndrome(13.92%)> liver and stomach qi stagnation syndrome(13.76%)> spleen-stomach damp heat syndrome(10.08%)> stomach yin deficiency syndrome (4.00%).3. The main TCM syndrome of atrophy of mucosal is liver and stomach qi stagnation syndrome, while the light degree is mainly of liver and stomach qi stagnation syndrome, the moderate and severe degree is mainly of stomach meridian of blood stasis syndrome. The main TCM syndrome of intestinal metaplasia is stagnated heat of liver and stomach syndrome, while light degree is mainly of liver and stomach qi stagnation syndrome, and the moderate degree is mainly of stomach yin deficiency syndrome, and the severe atrophy is mainly of stomach meridian of blood stasis syndrome. The main TCM syndrome of dysplasia is stagnated heat of liver and stomach syndrome, while light degree is mainly of stagnated heat of liver and stomach syndrome, and the severe degree is mainly of stomach meridian of blood stasis syndrome.4. The pathological changes of liver and stomach qi stagnation syndrome, spleen-stomach deficiency syndrome, stagnated heat of liver and stomach syndrome, spleen-stomach damp heat syndrome and stomach yin deficiency syndrome are commonly seen with mild atrophy and mild intestinal metaplasia; stomach meridian of blood stasis syndrome occurred more often with moderate atrophy and mild intestinal metaplasia. Dysplasia of gastric mucosal of different syndromes is mostly mild degree. Spleen-stomach deficiency syndrome, spleen-stomach damp heat syndrome and stomach meridian of blood stasis syndrome may also be found in the moderate and severe dysplasia.5. The gastric angle is the most selected part which we choose. In this part of the pathological changed, the main TCM syndrome of atrophy of mucosal is stomach yin deficiency syndrome. The main TCM syndrome of intestinal metaplasia is stagnated heat of liver and stomach syndrome. The main syndrome of light degree of dysplasia which is more is stagnated heat of liver and stomach syndrome. The pathological changes of liver and stomach qi stagnation syndrome, spleen-stomach deficiency syndrome, stagnated heat of liver and stomach syndrome and stomach yin deficiency syndrome are commonly seen with mild atrophy and mild intestinal metaplasia; spleen-stomach damp heat syndrome occurred more often with mild atrophy and moderate intestinal metaplasia; stomach meridian of blood stasis syndrome occurred more often with moderate atrophy and mild intestinal metaplasia. Dysplasia of gastric mucosal of different syndromes is mild degree.6. It is the standard pathological site, that is gastric angle, the greater curvature of antrum, antral gastric juice, the greater curvature of the gastric body and small bending the body of stomach. Gastric angle is commonly seen with mild atrophy, mild and severe intestinal metaplasia and mild dysplasia. The greater curvature of antrum is commonly seen with mild atrophy, moderate intestinal metaplasia and mild dysplasia. Antral gastric juice is commonly seen with mild atrophy, mild intestinal metaplasia and mild dysplasia. The greater curvature of the gastric body is commonly seen with mild atrophy, mild and moderate intestinal metaplasia. The small bending the body of stomach is commonly seen with mild or moderate atrophy and intestinal metaplasia.ConclusionCAG occurs more often among people older than 50 years old. The main TCM syndrome of CAG is spleen-stomach deficiency syndrome. There are correlations between different TCM syndromes and pathological changes. Atrophy of gastric mucosal occurs more often with liver and stomach qi stagnation syndrome, intestinal metaplasia and dysplasia occurs more often with stagnated heat of liver and stomach syndrome. The pathological site in the stomach which is the most is gastric angle and gastric antrum lesser, while the light is the greater curvature of the gastric body.
