| ObjectiveTo observe the effects of acupuncture and health therapy on the depression state,functional activity of brain regions,and serum markers in patients with post-stroke depression,and to explore the central mechanism of acupuncture and health therapy in improving the depression state of post-stroke depression patients.MethodsThe collected 24 patients with post-stroke depression(PSD)were divided into the control group and the acupuncture-health group according to the random number table method.On the basis of routine treatment in the rehabilitation department,acupuncture(cluster acupuncture at scalp points + psychological rehabilitation training)was performed.Both groups were evaluated with Hamilton Depression Scale(HAMD),Fugl-Meyer Motor Function Assessment Scale,and Barthel Index Scale before treatment and after 4 weeks of treatment;serum markers of brain-derived neurotrophy Factor(Brain-derived neurotrophic factor,BDNF),Vascular endothelial growth factor(Vascular endothelial growth factor,VEGF),5-hydroxytryptamine(5-Hydroxytryptamine,5-HT),Dopamine(Dopamine,DA),Glutamate(Glutamate,Glu)detection;functional magnetic resonance imaging(f MRI)scans.Statistical analysis of evaluation and detection data and scanning data is performed by using relevant software.Result1.Behavioral scale evaluation results: The Fugl-Meyer and Barthel scores of the two groups of patients after treatment were higher than those before treatment,and the acupuncture group was better than the control group,with statistical significance(P < 0.05).The HAMD scores of the two groups after treatment were lower than those before treatment,and the acupuncture-kang group was better than the control group,with statistical significance(P < 0.05).2.Serum detection index results: the levels of BDNF,VEGF,5-HT,and DA in the two groups after treatment were higher than those before treatment,and compared with the control group,the levels were significantly increased(P < 0.05).The levels of Glu in the two groups of patients after treatment Compared with the control group,the levels were significantly lower than those before treatment(P < 0.05).3.f MRI study results: After treatment,there were significant differences in brain regions between the two groups of Re Ho and ALFF(P < 0.01).After treatment,the ALFF values of the right cingulate gyrus and caudate nucleus in the control group increased,while the ALFF values of the lower cerebellum,the left inferior temporal gyrus,and the right precentral gyrus decreased.The ALFF value of gyrus increased,and the ALFF value of temporal pole superior temporal gyrus,left hippocampus,and left cingulate gyrus decreased;Leaf ALFF value increased.The Re Ho value of the right middle temporal gyrus in the control group increased after treatment;the Rohe value of the right lingual gyrus increased after treatment in the acupuncture group;after treatment,the caudate nucleus,hippocampus,left precuneus,cingulate gyrus,The Reho value of the left lingual gyrus and the left middle temporal gyrus increased,and the Reho value of the left cerebellar peduncle 2 and the left middle frontal gyrus decreased.4.Correlation analysis results: There was a negative correlation between the cingulate gyrus and the HAMD scale score in the brain region with the difference in Reho value(r=-0.421P=0.039);the precuneus in the brain region with the difference in ALFF value was negatively correlated with the score on the HAMD scale(r=-0.421P=0.039).r=-0.48,P=0.03);there was a positive correlation between the precentral gyrus and the Barthel index scale score in the difference of ALFF value(r=0.57,P=0.004);the hippocampus of the difference in Reho value was positively correlated with BDNF(r=0.48,r=-0.48,P=0.03).P=0.02).Conclusion1.Acupuncture and health therapy can improve the depressive state of PSD patients,and have certain curative effects on patients’ motor function and daily living ability,which may be related to the regulation of neurotransmitter content by acupuncture and health therapy,promoting neuroplasticity and reducing excitotoxicity;2.Through the analysis of f MRI scan results,acupuncture can improve the left superior parietal gyrus,left precentral gyrus,left posterior central gyrus,precuneus ALFF value and caudate nucleus,left precuneus,cingulate gyrus,left lingual gyrus,left middle temporal gyrus Reho value,the central mechanism of acupuncture and health therapy in the treatment of PSD may be related to the enhanced activity of autonomic nerve function in these brain regions. |
