Literature Review And Study On Parkinson Disease With TCM Treatment

ObjectivesThe objectives of this study are to analyze the literature publications in the past 10 years for Parkinson disease(PD)treatment using traditional Chinese medicine(TCM).The study of literature paper quality for each publication is carried out through the assessment of research background,methodology used,outcome and potential problems inherited.The aim is to study clinical symptoms,patterns of syndromes and TCM treatment of Parkinson diseases,in order to provide some references for future TCM treatment on PD,so to help patients to improve overall treatment efficiency together with western medicine,to prolong the progression of illness,and to reduce the side effects of western medicine,eventually to improve the life quality of patients.MethodsLiterature search covers China's large-scale Bibliographic Database CNKI,together with tracing back and manual searching.The on-line search spans publications related to clinical symptoms,syndrome patterns,Parkinson disease,as well as TCM treatment for Parkinson disease,published in the period between January 2008 to December 2018.The key words used to search on line literature publications are "Parkinson disease" "tremor symptom" "TCM treatment"?The sample size in the study must be more than 30 cases.The literature information is collected using tables and graphs.Microsoft Office Excel 2016 is used for data collection and statistical analysis.The root cause analysis on PD and types of symptoms,treatment methods and the TCM medicines used are studied and analyzed.1.Literature review of randomized controlled clinical trials treatment for PDThe study on the quality of publications is carried out for all the publications searched,using the principles of clinical epidemiology and evidence-based medicine(EBM).For the data analysis and statistical study,Microsoft Office Excel 2016 is used.2.Literature review of on clinical symptoms,patterns of syndromes and TCM treatment for PDFor all the literature publications collected,extraction of information such as article title,author's name,clinical symptoms,syndrome patterns,principle of TCM treatment,prescribed herbs and duration of treatment etc.are carried out.Microsoft Office Excel 2010 software is used to create database on literature record tracking and statistical analysis.Results1.Literature review of randomized controlled clinical trials treatment for PDThrough advanced literature search,29 out of 149 literature papers are selected for the study after eliminating those not meeting the inclusion criteria and applying exclusion criteria.Among them,13 papers are published in the national level journals which is 45%of total papers,while 3 of them are sponsored by the national foundations and 3 are sponsored by local foundations,which in total 21%of papers are sponsored by foundations.Among the 29 papers,there is 1 paper using computer generalized random controlled clinical study,while 23 papers use number generated random control which occupy majority of 80%.There are 3 papers using other randomized control methods,while 2 papers do not describe on the randomized methods.All of 29 papers use proper statistical methods for analysis.On the study of the effectiveness of treating PD using TCM,19 papers describe the total effectiveness of treatment for the study group and the controlled group.The average effectiveness of the study group is 85%which is higher than the controlled group of 65%.On the study of the side effects,only 8 papers describe them.The average of side effects for the study group is 10%which is lower than the controlled group of 25%.2.Literature review of on clinical symptoms,patterns of syndromes and TCM treatment of PDThere are 32 papers meeting requirements from the total of 149 literature papers,after applying exclusion criteria and inclusion criteria.Among these papers,there are total of 27 types of syndromes,the most frequent syndromes are yin deficiency of the liver and kidney,qi and blood deficiency,blood stasis and phlegm stasis.For the TCM treatment,the most frequent used methods are tonic liver and kidney,replenishing qi and nourishing the blood,removing phlegm,and promoting blood circulation and removing blood stasis.For the 32 papers which prescribe TCM medicines,there are total of 102 types of herbs are used.Among them,the most frequent used herbs are below 8 herbs:tonic blood type bai shao,shou di huang and dang gui,flatten liver wind type of herbs such as tian ma,gou teng and quan xie,improve yin and yang type of herbs as shan zhu yu,circulating blood and resolving stasis type of herbs such as chuan xuan,other types of commonly used herbs are improving blood type of he shou wu,improving yin type of herbs gou ji zi and gui jia,improving yang type of herbs rou chong rong,improving qi type of herbs gan cao,circulating blood and resolving stasis type of herbs dan shen and niu qi,removing water retention type of herbs fu lin,flattening liver wind type of herbs di long and jiang chan.Therefore,improving blood,flattening liver wind,circulating blood and resolving stasis,and tonic yin and yang types of herbs are most commonly used TCM for treating PD.In summary,this study shows that TCM treatment is effective for PD,it can also reduce the side effect of western medicines,and bring the best benefit for PD patients,it is suitable for long term treatment.The main pathogenesis of Parkinson's disease is liver and kidney yin deficiency,qi and blood deficiency,blood stasis and phlegm stasis.Clinical treatment should be based on nourishing the liver and kidney,nourishing the qi and blood,removing phlegm stasis,and promoting blood circulation to remove blood stasis.The most frequent used TCM herbs are bai shao,shou di huang,dang gui,tian ma,shan zhu yu,quan xie,chuan xuan,gou teng,mainly belong to liver system,kidney system,heart and spleen system.ConclusionsThe conclusions of this study show that there are reasonably good standards for these literature publications on the randomized clinical study of TCM treatment for PD.Nealy half of them are published in national standard journals,majority of them use proper randomized and statistical methods.In the effectiveness comparison,the overall effectiveness for patients using TCM treatment is higher than only using western medicine treatment,while the side effect is lower.In the study on clinical symptoms,patterns of syndromes and TCM treatment of PD,these are most common types of PD which are yin deficiency of liver and kidney,qi and blood deficiency,blood stasis and phellem stasis.The clinical treatment methods are tonic liver and kidney,improving qi and blood,removing sputum and resolving stasis,and circulating blood and resolving stasis.The most frequent used TCM herbs are bai shao,shou di huang,dang gui,tian ma,shan zhu yu,quan xie,chuan xuan,gou teng,mainly belong to liver system,kidney system,heart and spleen system.In conclusion,the clinical treatment of PD must be based on analyzing syndrome and using multiple methods and medicines.The main approaches are tonic liver and kidney,improving qi and blood,together with removing sputum,circulating blood and resolving stasis,invigorating spleen and soothing liver,in consideration of individual situation.The main purposes are to strengthen up the body resistance and eliminate illness,soothing up qi and blood,yin and yang,therefore to achieve the satisfactory results of TCM treatment.
