Background and objectiveFemale pattern hair loss(FPHL)is a common female alopecia disease characterized by progressive and diffuse hair loss on the vertex and forehead. This can have a serious impact on women's mental health and social behavior.Low-level laser therapy(LLLT)is widely used in various clinical fields and various clinical trials have preliminarily verified its effectiveness in the treatment of alopecia.Instruments based on LLLT were approved by FDA in 2007 for the treatment of male androgen hair loss, and for female in 2010.Although some clinical trials have covered the female population, there is still a lack of a large sample of high-quality clinical trials to study the difference of efficacy and safety of low-energy laser therapy and other common non-surgical treatments for female pattern hair loss, especially in Asia. This study has further broadened the application prospect of low-level laser therapy for hair loss by comparing and evaluating the efficacy and safety of low-level laser therapy with minoxidil and mesodermal therapy(Platelet-rich Plasma,PRP)for treatment of the female pattern hair loss. The advantages and necessity of multi-therapy combination have been further explored either.Methods1.Low-level light therapy-minoxidil combination versus either alone for treatment of female pattern hair lossNinety Ludwig's type ?-? FPHL patients aged 20-50 were randomly divided into three groups:LLLT group(A),5%minoxidil group(B)and combined therapy group(C).Patient of LLLT group was given a household helmet-type instrument with specific physical parameters(650nm,5mW).The combined group has used both LLLT and topical 5%minoxidil.All patients were going to a treatment period of 6 months. The differences in hair density, hair diameter and scalp's oil-secretion were all evaluated.2.Low-level light therapy-PRP injection combination versus either alone for treatment of female pattern hair lossSixty Ludwig's type ?-? FPHL patients aged 20-50 were randomly divided into three groups:LLLT group(A),PRP injection group(B)and combined therapy group(C).Patient of LLLT group was given a household helmet-type instrument with specific physical parameters(650nm,5mW).The combined therapy group had used both LLLT and PRP injection. All patients were going to a treatment period of 6 months and will be record the hair density and diameter before and after the treatment. All results were statistically analyzed.Results1.Low-level laser therapy combined with minoxidil treatmentAfter 6 months treatment, the hair density, hair diameter and oil secretion of three groups of patients were significantly improved. No adverse events or side effects were reported. The effect of reducing oil secretion was better in LLLT group and combined therapy group than that of minoxidil group(P<0.05).The combined therapy group was superior to LLLT group and minoxidil group in improving hair diameter and hair density.2.Low-level laser therapy combined with PRP injectionThree groups had been observed to improve hair density and hair diameter levels over a period of six months. The LLLT group outperformed PRP injection group in increasing hair density, yet PRP injection group was superior in hair thickening(p<0.05).The combined therapy group had more comprehensive curative effect and higher overall germinal range than mono-therapy.There was reported cases of posterior scalp swelling and pain after treatment in PRP injection group, these symptoms had improved without any treatments.ConclusionLow-level laser therapy is an important non-surgical treatment for female partten hair loss.It can significantly improve the hair density and reduce scalp's oil production. When combining LLLT with 5%minoxidil or PRP injection, much more improvement of hair density and hair diameter levels can be achived.Compared with other non-surgical treatments,low-level laser therapy has showed definite effect, few side effects, convenient and non-invasive use experience, and further improvement of the efficacy when combined with other therapies. In a word,it can be widely used in the non-surgical treatment of female pattern hair loss.
