ObjectiveThis study aims to research on lumbar muscle strain treatment through clinical application of Sinew acupuncture,ordinary acupuncture and traditional Chinese medicine(TCM).At present,we are unclear about the different curative effects of these 3 methods.Therefore this research aims to compare the effectivenss of the above three treatment methods in healing lumbar muscle strain.And to find out if the Sinew acupuncture had relatively better curative effects than ordinary acupuncture and traditional Chinese medicine(TCM).MethodsIn order to study the clinical effect of Sinew acupuncture on lumbar muscle strain,this small sample study used literature search to draw on the research experience of others,and then set the research method including the research object and detailed research plan.Research objects were randomly divided into the Sinew acupuncture group,control group 1 and control group 2 with 30 cases each.Sinew acupuncture was applied in the experimental group,in which the doctor would identify nodules,pain points and comfortable points as acupoints for Sinew acupuncture,with emphasis on pain points as key selected acupoints,taking 4 acupoints from strained waists to prescribe acupuncture 4 times within a four-week period.Controlgroup 1 was treated with ordinary acupuncture methodology.Mild reinforcing-reducing method(?????),was performed on both sides of the Kidney Shu(BL23)and the Large Intestine Shu(BL24),4treatments in 4 weeks.Control group 2 was prescribed with TCM compound Du Huo Ji Sheng Tang,10g/time a day,5 times/week,for a total of 4 weeks.The above three groups of patients were also required to do 3 assigned basic lumbar muscle exerciseson their own to strengthen their lumbar muscles.The data was collected via questionnaires from the three groups of patients with lumbar muscle strain before and after treatment,they include(1)Visual Analogue Scale(VAS);(2)Numerical Rating Scale(NRS).This item is only to repeatedly seek the patients' confirmation on the data of the Visual Analogue Scale(VAS)above,does not need to be summarized;(3)Daily living ability assessment SF Health Survey(SF-36);(4)waist movement range score;(5)Number of waist pain points.The location of pain points are informed by the patients,and the doctor confirmed the location of pain points by the pressing technique;(6)Comprehensive Efficacy Assessment:this was formulated according to the“Traditional Chinese Medicine Industry Standard of the People' s Republic of China-Traditional Chinese Medicine Orthopedic Traumatology Diagnostic Efficacy Standard”.Data collected from the clinical studies are maintained and analysed based on the multivariate statistical analysis method(SPSS22.0).A database was established for analysis of various purposes.The main points of the clinical research results are summarized for sharing purpose.ResultsThe ages of the three groups obeyed the normal distribution(Shapiro-Wilk.P>0.05),and the homogeneity of variance test showed homogeneity of variance(P=0.527>0.05).A one-way analysis of variance)(LSDmethod was used for comparison.F=0.28 and P=0.76>0.05,indicating that the ages of the three groups of patients were not significantly different.Chi-square test of the three groups showed that ?2=0.11,P=0.95>0.05,indicating that there was no significant difference in gender between the three groups.In terms of lumbar muscle strain pain Visual Analogue Scale(VAS),the Before Treatment VAS scores were not significantly different among the three groups,and are comparable.The After Treatment VAS scores of the three groups were compared,and the scores in the three groups obeyed the normal distribution(Shapiro-Wilk,P=0.117,0.092,0.471>0.05),the variances are homogeneous(P=0.36>0.05).All groups had P=0.000<0.01 in the Two-paired sample t test to compare the changes in the same group before and after treatment,indicating that all three groups had pain relieved to a certain extent after treatment.Single factor analysis of variance(LSD method)results of F=5.86,P=0.004<0.01 indicates that the VAS scores of the three groups were significantly different after treatment,calling for further intra-group comparison in the three groups(Mann-Whitney).The Sinew acupuncture group compared with the common acupuncture group,P=0.003<0.01,compared with the traditional Chinese medicine group,P=0.004<0.01,indicating that the pain relieving effect in Sinew acupuncture group is better than that in ordinary acupuncture and traditional Chinese medicine.Assessment of daily living ability(SF-36):There was no significant difference in the daily living ability assessment of the three groups of patients before treatment,thus the patients are suitable for comparison purpose.The waist pain points data in the three groups after treatment did not obey normal distribution(Shapiro-Wilk,all P<0.05).P=0.000,0.000,0.000,0.000,0.000,0.002,0.006,0.000:0.000,0.000,0.000,0.000,0.000,0.000,0.001,0.000:0.000,0.000,0.000.0.000,0.000,0.005,0.006,0.001<0.01 were returned in the non-parametric test(Wilcoxon method)of two related samples to compare the changes in the same group before and after treatment,indicating that the three groups had improvements in the daily living ability score to a certain extent after treatment,that is,the patients' quality of life improved.Further non-parametric tests(Kruskal-Wallis)were used to compare the efficacy among the three groups,and there were significant differences in the three items of physiological function,occupational function,and physical pain,X2=11.051,6.467,12.028,all have P<0.05,indicating that the three therapies have significant efficacy differences in the three sub-items of physiological function,occupational function,and physical pain;Further intra-group pair-wise comparisons in the three groups were performed(Mann-Whitney).In the physiological function sub-item,Sinew group versus ordinary acupuncture group with P=0.022<0.05,Sinew group versus traditional Chinese medicine group with P=0.002<0.01,indicating Sinew is better than ordinary acupuncture and traditional Chinese medicine in improving the physiological functioning of the patients.In the occupational function sub-item,Sinew group versus ordinary acupuncture group with P=0.007<0.01,Sinew group versus traditional Chinese medicine with P=0.239>0.05,indicating Sinew is better than ordinary acupuncture in improving the occupational functioning of the patients.Sinew and traditional Chinese medicine perform similarly in this sub-item.In the physical pain sub-item,Sinew group versus ordinary acupuncture group with P=0.001<0.01,Sinew group versus traditional Chinese medicine group with P=0.006<0.01,indicating physical pain is better improved in Sinew group than in the other two groups.Non-parametric tests(Kruskal-Wallis)on the five aspects of overall health,energy level,social function,emotional function and mental health returned with ?2=3.878,0.894,1.161,2.135,3.563 and all P>0.05,indicating that the three therapies have basically the same effect in improving patients' overall health,energy,social function,emotional function and mental health.Waist motion range score:There was no significant difference in the waist motion range score between the three groups of patients before treatment,thus the patients are suitable for comparison purpose.Comparing the waist motion range scores in the three groups after treatment by single factor analysis of variance(LSD),F=11.85,P=0.000<0.01,indicating that the three therapies have significant differences in improving waist range scores;Further intra-group pair-wise comparisons in the three groups were performed(Mann-Whitney).Sinew group versus ordinary acupuncture group with P=0.027<0.05,Sinew group versus the traditional Chinese medicine group with P=0.000<0.01,indicating that improvement in this aspect in the Sinew group is better than that in the other two groups.Waist pain points:There was no significant difference in number of waist pain points between the three groups of patients before treatment,thus the patients are suitable for comparison purpose.The number of waist pain points in the three groups after treatment did not obey the normal distribution(Shapiro-Wilk,P=0.000,0.001,0.007<0.05).The changes are all P<0.05 in the non—parametric test(Wilcoxon method)of two related samples to compare the same group,indicating that the numbers of waist pain points were reduced by a certain extent after treatment in all three groups.Non-parametric test(Kruskal-Wallis)to compare the efficacy among the three groups returned with ?2.52,P=0.28>0.05,indicating that there was no significant difference in reducing numbers of waist pain points among the three groups of patients,that is,the efficacy of the three treatments in treating waist pain points were basically the same.Comprehensive efficacy evaluation:In Sinew group,the total after treatment effective case count was 29.In ordinary acupuncture group,the total effective case count was 28,and the total number of effective cases in the Chinese medicine group was 25.Conclusion1.Clinical treatments using the three methods,Sinew acupuncture,ordinary acupuncture and traditional Chinese medicine showed that all 3 methods have curative effect.2.The research results of this small sample study of 90 samples showed that Sinew acupuncture is better than ordinary acupuncture and traditional Chinese medicine in the treatment of lumbar muscle strain in several aspects.They are reflected in the lumbar muscle strain pain Visual Analogue Scale(VAS)scores,physiological function sub-item and physical pain sub-item of the assessment of daily living ability(SF-36)and the waist movement range score.3.Comprehensive efficacy evaluation:In Sinew group,the total after treatment effective case count was 29 and the total efficiency is 96.7%.In ordinary acupuncture group,the total effective case count was 28 and the total efficiency is 93.3%.The total number of effective cases in the Chinese medicine group was 25 and the total efficiency is 83.3%.The overall results indicated that the immediate curative effect of Sinew acupuncture is better than the other 2 treatment methods.
