Clinical Value Of Serum MiR-200c-3p In The Diagnosis Of EC

Objective: To learn the incidence and clinical and pathological features of endometrial cancer(EC),and to explore the significance of target microRNA(miRNA)in its diagnosis.Methods: We analyzed 251 hospitalized patients in a retrospective way with newly diagnosed EC and were seeking for surgical treatment.Their basic clinical and pathological data were recorded.Simultaneously,we also collected serum of 16 EC patients and other 8 normal controls,using microarray chip technology and real-time quantitative PCR(qRT-PCR)to screen and verify differentially expressed miRNA molecules.Results:(1)Most of EC patients were aged between 45 and 65 years,and the main cause of EC was correlated with continued exposure to estrogen and no progesterone resistance.Risk factors included obesity,high blood pressure,diabetes mellitus,etc;(2)Univariate analysis showed that extrauterine metastasis of EC was correlated with pathological type,histological grade,myometrial invasion,lymph vascular space invasion,cervical stromal invasion,peritoneal cytology,PR,CA125 and CA199 expression(P<0.05);Multivariate analysis showed that type?or mixed-type EC,histological grade G3,depth of myometrial invasion ?1/2,positive lymphatic vascular space invasion and peritoneal cytology were independent risk factors for extrauterine metastasis of EC(P<0.05);(3)Preoperative serum CA125 and CA199 were closely associated with many pathological risk factors of the development of EC,such as clinical stage,histological grade,myometrial invasion,lymph vascular space invasion,cervical stromal invasion,adnexa metastasis,lymph node metastasis and peritoneal cytology(P <0.05);(4)To differentiate early or late stage of EC,the area under the ROC curve of serum CA125 was(AUC = 0.729,95% CI: 0.614~0.845),wth 82.6% sensitivity and 55.8% specificity,and the difference was statistically significant(P = 0.0003);the area under the ROC curve of serum CA199 was(AUC = 0.694,95% CI: 0.561~0.827),with 60% sensitivity and 23% specificity,and the difference was also statistically significant(P=0.0037);(5)Compared to healthy controls,miR-200c-3p expression was significantly lower(P<0.05)in the serum of type?or stage?EC patients,whereas the expressions of miR-299-3p and CA125 in the serum of EC patients and healthy controls were not significant(P?0.05);(6)The expression of CA125 in the serum of stage?EC patients and healthy controls had no significant difference(P?0.05);(7)As for the diagnosis of EC,the area under the ROC curve of serum CA125 was(AUC=0.505,95% CI: 0.24~0.78),while the area under the ROC curve of serum miR-200c-3p was(AUC = 0.813,95% CI: 0.58~1.05),with 71.4% diagnostic sensitivity and 23.0% specificity,and the difference was statistically significant(P=0.024).Conclusion: Serum miR-200c-3p showed specific low expression in type?or stage?EC patients,and is expected to become a novel biomarker for the early diagnosis of EC.
