Improvement Of Magnetic Controlled Capsule Endoscopy And Its Clinical Value

Objective: The aim of this study is to improve the detection rate of magnetic controlled capsule endoscopy and investigating its clinical value in the diagnosis of stomach and small intestine diseases in minors.Methods: 77 patients were enrolled in the first part.During the examination,we observed six gastric landmarks by changing patients' positions(left lateral,supine,right lateral,knee-chest and sitting positions).The complete visualization rate of each gastric landmarks and the complete observation rate of the whole gastric mucosa were analyzed from single position and a variety of body position combinations.Second,based on the improved method,84 cases of minor patients underwent stomach and small intestine examination by magnetic controlled capsule endoscopy.The diagnostic yield and sensitivity of MCCE in minors were analyzed.Results: Among the three-position combinations,the fundus was better observed in left lateral + supine + right lateral combination compared with supine + knee-chest + sitting combination and supine + right lateral + kneechest combination(98.7%,p<0.01),but it had the lowest completion rate of angulus visualization(58.4%,p<0.01).The five-position combination had the highest completion rate for total stomach observation(94.8%,p<0.01).The main indications of magnetic controlled capsule endoscopy in minors were Crohn's disease,gastrointestinal bleeding and abdominal pain.The diagnostic yields of stomach and small intestine were 13.1% and28.6%,respectively,and the sensitivities were 100% and 96.0%,respectively.No adverse events occurred.Conclusion: Compared with the traditional three position combination,the five-position combination can effectively improve the completion rate of the whole stomach observation.And as a safe,effective and well tolerated endoscopic method,magnetic controlled capsule endoscopy had a prospective clinic value for the diagnosis of stomach and small bowel diseases in minors.
