Purpose:To analyze clinical manifestations of eczematous psoriasis,to investigate the roles of multiple cytokines and B cell subsets in eczematous psoriasis.Method:6 patients diagnosed on out-patient and in-patient were selected as case group,meanwhile 16 psoriasis vulgaris were selected as control group,Electrochemistry was used to detect IL-1?,IFN-?,IL-4,IL-5,IL-13,IL-17 A in the peripheral blood as well as urine of the patients,PBMC was desected,with eleven-color cytometry immunoglobin,CD19 and CD20 recognized as markers of B cells and Ig D?CD27?CD24?CD38 were selected as anchored marker to distinguish B cells based on phenotypes.Immunohistochemistry was used to detect B cell expression in the skin.Results:We found that there was significant difference in expression of serum IL-17A?IFN-? between eczematous psoriasis and psoriasis vulgaris.The expression of them wwas not increased in eczematous psoriasis;Serum IL-4?IL-5 levels in eczematous psoriasis patients were significantly lower than in psoriasis vulgaris patients,while higher than those in psoriasis vulgaris.The expression of urine IL-5 in eczematous psoriasis was significantly higher than those in AD as well as psoriasis vulgaris.And the expression of IL-13 was significantly lower than those in AD patients as well as lower limit.There was no difference found in the expression of IL-17,IL-4 and IL-1? in urine and IFN-? was not detected in urine.Propotion of CD19+ cells in peripheral blood was significantly lower in eczematous psoriasis than in AD,while the difference between psoriasis vulgaris and AD was more significant.Meanwhile,there was significantly increase of CD20+ cells in skin lesions of eczematous psoriasis,but there was no apparent specificity in B cell phenotype.Conclusion:Cell balance of eczematous psoriasis is switched from Th1 to Th2,inducing increase of Ig E or EOS in eczematous psoriasis,causing eczema-like change.To sum up,we propose that B cells play an important role in the pathogenic mechanisms of eczematous psoriasis.
